Technetium-99m-MAG3 in comparison to iodohippurate

Description
For detection of renal disorders radioiodinated o-iodohippurate (OIH) is increasingly replaced by 99mTc-MAG3. However, for interpretation of clinical findings the pharmacokinetic differences between the two radiopharmaceuticals must be taken into account. Thus, simultaneously steady-state-clearance measurements resulted in a 1.5-fold higher clearance of OIH than of 99mTc-MAG3. Pharmacological loading of the tubular transport system by p-aminohippurate revealed a significantly higher suppression of the 99mTc-MAG3 clearance than of the OIH clearance which indicates a lower affinity of the 99mTc complex to the receptors of the tubular cell; however, these findings seems not to be of clinical relevance even in cases of severely impaired renal function. The new radiopharmaceutical offers all advantages of the 99mTc label such as low radiation exposure, high image resolution as well as permanent availability, and is suited not only for scintigraphic imaging but also for quantitative clearance measurements. Abstract (German)
In der nuklearmedizinischen Nierenfunktionsdiagnostik wird radiojodierte Hippursaeure (OJH) zunehmend durch 99mTc-MAG3 ersetzt. Dabei muessen zur Interpretation der klinischen Ergebnisse die pharmakokinetischen Unterschiede zwischen den beiden Radiopharmaka beruecksichtigt werden. Simultane Clearancemessungen unter Gleichgewichtsbedingungen ergaben eine um den Faktor 1,5 hoehere Clearance von OJH als von 99mTc-MAG3. Die pharmakologische Belastung des tubulaeren Transportsystems mit p-Amino-Hippursaeure fuehrte zu einer signifikant staerkeren Abnahme der 99mTc-MAG3-Clearance als der OJH-Clearance. Dies weist auf eine geringere Affinitaet des 99mTc-Komplexes zu den Rezeptoren der Tubuluszelle hin, jedoch scheint dies auch bei Patienten mit eingeschraenkter Nierenfunktion klinisch nicht relevant zu sein. Das neue Radiopharmakon bietet alle Vorteile der 99mTc-Markierung wie geringe Strahlenbelastung, hohe Abbildungsqualitaet sowie staendige Verfuegbarkeit und eignet sich nicht nur zur Szintigraphie, sondern auch fuer quantitative Clearancemessungen.
