Selected safety-related occurrences reported in November and December 1974

Description
Of the occurrences reported during November and December 1974, five are reviewed because of their uniqueness and/or general interest to reactor operations. At Dresden 2 and Quad Cities 2, cracks from internal stress-assisted corrosion were discovered. A main-steam-line isolation valve leaked intermittently at the La Crosse boiling-water reactor (BWR). Cover gas from the quench tank at Maine Yankee leaked into the containment during shutdown work. Peach Bottom 2 was depressurized twice by failure of safety relief valves. Somewhat less than 6000 gal of primary water leaked from the feedwater system at Quad Cities 2. (U.S.)
