Description
Purpose: The clinical evaluation of digital subtraction angiography with carbon dioxide using a newly developed low-tech CO2-dosage- and injection system. Method and patients: The hand-held system (CO2 angio set) consists of a dosage chamber in connection with a special stopcock to apportion the gas. By optimising injection volume and pressure steady gas flow characteristics are approximated. CO2 arteriographies were performed on 185 patients. Main indications were renal insufficiency and a history of adverse reactions to iodinated contrast media. In patients with femoral connula access, catheterless reflux angiography was performed. Results: The injection system provided complete and coherent visualisation of the abdominal aorta, visceral, pelvic, and lower limb arteries via catheter (71 cases) or via femoral cannula using reflux technique (114 cases). Stenoses, occlusions, and collaterals were assessable. Employing the gas reflux over the aortic bifurcation bilateral run-off studies up to the calf trifurcation were performed via unilateral femoral cannula. Use of a dedicated stacking software improved image quality of distal femoral, popliteal and calf arteries. Conclusion: The CO2 management system allows adequate imaging of the arteries below the diaphragm. Ease and safety of use and low costs are advantageous. (orig.)
Abstract (German)
Ziel: Die klinische Erprobung der digitalen Subtraktionsangiographie mit Kohlendioxid unter Verwendung eines neuentwickelten, einfachen Gasdosier- und Injektionssystems. Methode und Patienten: Eine Dosierkammer mit einstellbarem Volumen in Verbindung mit einem Spezial-Hahn (CO2-Angio-Set) portioniert das Gas. Durch Optimierung von Injektionsvolumen und -druck wurde eine konstante Ausstroemcharakteristik angenaehert. Das System wurde bei insgesamt 185 Patienten zur arteriellen Gefaessdarstellung mit CO2 eingesetzt. Hauptindikationen waren Niereninsuffizienz oder Unvertraeglichkeitsanamnese gegenueber jodhaltigem Kontrastmittel. Bei Patienten mit einem femoralen Kanuelenzugang wurde eine katheterlose Reflux-Angiographie durchgefuehrt. Ergebnisse: Das Injektionssystem ermoeglichte eine vollstaendige und zusammenhaengende Kontrastierung der Bauchaorta, der Viszeral-, Becken- und Beinarterien ueber einen Katheter (in 71 Faellen) oder in Refluxtechnik ueber eine femorale Kanuele (in 114 Faellen). Stenosen, Verschluesse und Kollateralkreislaeufe wurden auswertbar dargestellt. Aufgrund des Gas-Refluxes ueber die Aortenbifurkation war mit einer einseitigen femoralen Kanuelenpunktion eine beidseitige Becken-Bein-Darstellung bis zur Unterschenkeltrifurkation moeglich. Im distalen Oberschenkel-, Knie- und Unterschenkelbereich waren Bildqualitaetsverbesserungen durch eine elektronische Einzelbildadditions-Software zu erzielen. Schlussfolgerung: Das Gasdosier- und Injektionssystem erlaubt eine adaequat beurteilbare Darstellung von Arterien unterhalb des Zwerchfells.
