Analysis of unusual events in the use of radioactive materials in Germany
Description
In the Federal Republic of Germany all safety-related events in the use and transportation of radioactive materials as well as in the operation of accelerators are registered. The analysis of these events shows the causes and lessons learned from it. Besides the summary of all unusual events occured in Germany during the last 7 years it is given a special insight into the events in the field of gamma radiography. The analysis of several technically caused deviations from normal operating course resulted in the improvement of special details of the devices. However, the technical failures were not the most frequent reasons for unusual events, but the incorrect behaviour of workers that means human errors. (orig.)
Abstract (German)
In Deutschland werden sicherheitstechnisch bedeutsame Ereignisse beim Umgang mit radioaktiven Stoffen, Betrieb von Beschleunigern und bei der Befoerderung radioaktiver Stoffe bundeszentral zusammengefasst. Die Analyse dieser Ereignisse zeigt die Ursachen und laesst Rueckschluesse auf vermeidbare Fehler zu. Neben der Gesamtuebersicht ueber die besonderen Vorkommnisse der letzten 7 Jahre und einer Analyse der Ursachen wird ein spezieller Einblick in die Untersuchungen der besonderen Vorkommnisse auf dem Gebiet der Gammaradiographie gegeben. Die Ursachenanalyse einiger technisch bedingter Abweichungen vom normalen Arbeitsablauf trug zur Verbesserung technischer Details an Geraeten bei. Allerdings waren nicht die technischen Defekte die Hauptursache besonderer Vorkommnisse auf diesem Gebiet, sondern unvorschriftsmaessiges Verhalten. (orig.)Additional details
