Description
In former times Pneumocystis carinii (PC) primarily affected premature infants. More recently, it has proved to be one of the major infectious complications among immunocompromised patients, particularly those with AIDS. The most frequent radiographic feature is diffuse, bilateral alveolar and interstitial perihilar shadowing, spreading in a butterfly pattern. The typical findings may mimic other infectious diseases, but the presence of large pleural effusions of hilar adenopathy is evidence against the diagnosis of PC-pneumonia. (orig.)
Abstract (German)
Die Pneumocystis-Carinii-Pneumonie, die in frueheren Jahrzehnten als Erreger der kindlichen Plasmazellpneumonie ein diagnostisches und therapeutisches Problem darstellte, hat als opportunistische Infektion bei immunsupprimierten Transplantatempfaengern und Tumorpatienten sowie bei AIDS-Kranken neue Bedeutung erlangt. Das typische Roentgenbild einer interstitiellen Pneumonie ist durch die mehr chronischen Verlaufsformen und die Mischinfektionen eher selten geworden. Haeufigster Roentgenbefund sind bilaterale, diffuse, perihilaer und basal betonte Infiltrate, die nur anfangs einen ueberwiegend interstitiellen Charakter haben. Atypische Befunde koennen fast jedes Bild imitieren. Pleuraerguesse und Lymphadenopathien sind selten und sprechen gegen eine reine Pneumocystis-Pneumonie. (orig.)Additional details
