Expanding implant indications in patients treated with radiotherapy thanks to advances in irradiation techniques
Description
The adverse effects of external radiotherapy treatment for head and neck cancer are manifold. In addition, resective surgery results in loss of substance, disrupting masticatory and aesthetic functions, with considerable psychosocial and functional impact. These diverse consequences make prosthetic rehabilitation in these patients more complex. Although dental implants offer an effective rehabilitation solution for many people, the question of osseointegration in irradiated bone calls for caution. There have been major advances in irradiation techniques, aimed at improving tumor management and preserving surrounding tissue. The aim of this thesis is therefore to determine the real impact of radiotherapy on osseointegration, thanks in particular to the recent techniques used, and on the survival of dental implants in these patients. Abstract (French)
Les effets nefastes du traitement par radiotherapie externe des cancers de la tete et du cou sont multiples. Par ailleurs, la chirurgie resectrice entraine une perte de substance perturbant ainsi les fonctions masticatoires et esthetiques, ceci ayant pour resultat un impact psychosocial et fonctionnel considerable. Ces diverses consequences rendent la rehabilitation prothetique chez ces patients plus complexe. Bien que les implants dentaires offrent une solution de rehabilitation efficace pour de nombreuses personnes, la question de l'osteointegration dans un os irradie engage a la prudence. Les techniques d'irradiation ont connu d'importantes avancees visant a ameliorer la prise en charge tumorale et la preservation des tissus environnants. L'objectif de cette these est ainsi de determiner l'impact reel de la radiotherapie vis-a-vis de l'osteointegration, notamment grace aux recentes techniques utilisees, et sur la survie des implants dentaires chez ces patients.
