Positron emission tomography for quantitative imaging of regional metabolism in the brain

Description
By means of positron emission tomography (PET) the distribution of certain isotopes in transverse sections through the body is visualized and the concentration of labelled tracers in the tissue quantified. Various biochemical and physiological processes in the brain can be followed by positronemitting radionuclides and compounds labelled thereby. Of these methods the measurement of glucose metabolism with 18fluorodeoxyglucose, of oxygen consumption with 15oxygen and of blood flow with 15O-carbon dioxide or 18fluoromethane are of greatest clinical relevance. These techniques demonstrated increases in metabolic rate after functional activation of respective brain regions and revealed local metabolic disturbances after different pathological changes of the central nervous system. With other labelled compounds protein synthesis, distribution of specific receptors, damage of the blood brain barrier, blood volume and regional uptake of drugs can be studied. PET thereby provides an insight in the biochemistry and the function of the brain which is at present not feasible with any other technology. (orig.)
Abstract (German)
Mittels Positronen-Emissions-Tomographie (PET) kann die Verteilung bestimmter Isotope im Koerper im Schnittbild dargestellt und die Konzentration markierter Tracer im Gewebe gemessen werden. Verschiedenartige biochemische und physiologische Parameter im Gehirn koennen mit positronenemittierenden Nukliden und damit markierten Verbindungen dreidimensional und regional verfolgt werden, wobei der Messung des Glucosestoffwechsels mit 18Fluordeoxyglucose, des Sauerstoffverbrauchs mit 15Sauerstoff und der Durchblutung mit markiertem 15O-Kohlendioxid oder 18Fluormethan die groesste praktische Bedeutung zukommt. Mit diesen Techniken wurden Stoffwechselsteigerungen bei funktioneller Aktivierung entsprechender Hirnregionen, aber auch lokale Stoerungen nach verschiedenartigen krankhaften Veraenderungen des Gehirns gefunden. Mit anderen markierten Verbindungen koennen Proteinsynthese, Rezeptorverteilung, Stoerung der Blut-Hirn-Schranke, Blutvolumen und regionale Aufnahme von Pharmaka dargestellt werden. Die PET erlaubt somit Einblicke in die Biochemie und Funktion des Gehirns, die derzeit mit keiner anderen Methode moeglich sind. (orig.)Additional details
