Radiosynoviorthesis: Radiochemical synovectomy in the treatment of rheumatoid arthritis

Radioactive substances are used increasingly in the local treatment of chronic arthritis. Yttrium-90, beta-emitter, is used mainly for the knee joints, rhenium-186 for shoulder, elbows, hips and ankle joints and erbium-169 for the small joints of the hands. It is mandatory that the radioactive material is injected stricly interarticulary and the joint should be splinted for 4 days without weight bearing to avoid leakage. The success rate for knee joints is 55%. Patients with an early state of arthritis have a higher chance of improvement. Complications are reported as 2%, one third of the cases develop a symphatic effusion for 3 days after the injection. The use of radionuclides can lead to chromosome aberrations and the use of such materials should be limited to patients not younger than 40 years of age. Based on the current experience radiosynoviorthesis seems to be a true alternative for surgical synovectomy especially for the knee joints. (orig.)
Abstract (German)
Zur Lokalbehandlung der chronischen Arthritis bieten sich radioaktive Substanzen an. Yttrium-90, ein Betastrahler, wird vornehmlich fuer die Kniegelenke eingesetzt, Rhenium-186 fuer Schulter, Ellenbogen, Hand, Huefte und Sprunggelenke und Erbium-169 fuer die kleinen Gelenke der Hand. Das radioaktive Material muss strikt intraartikulaer gegeben werden, und das Gelenk sollte 4 Tage danach ruhiggestellt werden, um eine Abwanderung des Materials zu vermeiden. Die Erfolgsquote fuer die Kniegelenke liegt bei 55%; Patienten mit Fruehstadien der Arthritis haben eine hoehere Besserungsrate. Komplikationen werden mit 2% angegeben, waehrend es in ca. einem Drittel der Faelle zu einem temporaeren Reizerguss nach Injektion kommt. Da es unter der Radionuklidanwendung zu Chromosomenaberrationen kommen kann, sollte diese Behandlungsmethode nicht bei Patienten, die juenger als 40 Jahre sind, eingesetzt werden. Nach den bisherigen Erfahrungen ist die Radiosynoviorthese eine echte Alternative zur Fruehsynovektomie, besonders der Kniegelenke. (orig.)Additional details
