Tumor scintigraphy using 123I-labelled estradiol in breast cancer - receptor scintigraphy

16-α-123I-Iodestradiol-17β (123I-E2) as a receptor-specific radiopharmacon was used for scintigraphic tumor detection in 62 patients suspected of breast cancer. The studies were performed as a multicenter trial to validate the method and to overcome methodical problems. A fast tracer elimination from the blood pool into the liver was seen, followed by biliary excretion allowing early imaging of the thorax due to low background activity but resulting in difficult imaging conditions of the abdomen. In 42 patients (30 carcinomas, 12 benign lesions) are overall sensitivity was 66%. Some patients with breast cancer showed focal or diffuse uptake in the area of primary lymph drainage without any clinical correlation. There was only one false-positive result in a receptor-negative primary carcinoma. The sensitivity of 123I-E2 in the detection of primary breast cancer or metastases and recurrences is low compared to mammography and other methods. Differentiation of malignant and benign tissue is difficult as both may have a positive ER status, in. Nevertheless, 123I-E2 scintigraphy is an in vivo imaging technique for the detection of breast cancer depending on the ER status and provides information about tumor localisation. It may become a specific method for the non-invasive diagnosis of the ER status and may be helpful in follow-up studies. As a receptor-specific agent 123I-E2 may give answers to questions of tumor heterogeneity and changes of the ER status during therapy. (orig./MG)
Abstract (German)
Im Rahmen einer Multizenterstudie (5 Zentren) wurde 16-α-123J-Jodoestradiol-17β (123J-E2) zur rezeptorspezifischen (E2-)Szintigraphie (planar und SPECT) bei 62 Patientinnen mit Mammakarzinom oder Verdacht auf Mammakarzinom eingesetzt, um methodische Probleme zu klaeren und die Methode zu validieren. Nach i.v. Gabe zeigte sich eine schnelle Elimination der Verbindung aus dem Blutpool und eine rasche Konzentration in der Leber mit nachfolgender Ausscheidung ueber die Gallenwege in den Darm, was zwar fruehe Aufnahmen bei niedrigem Background im Thoraxbereich ermoeglichte, die Beurteilung des Abdomens aber erschwerte. Bei 42 Patientinnen (30 Karzinome, 12 benigne Veraenderungen) ergab sich in Abhaengigkeit vom ER-Status eine Sensitivitaet von 66%. Bei einigen Patientinnen mit Tumoren zeigten sich fokale oder diffuse Mehrbelegungen in den Lymphabflussbereichen ohne ein klinisches Korrelat. Bei den bioptisch gesicherten Faellen wurde szintigraphisch lediglich ein falsch-positiver Befund erhoben. Aufgrund der geringen Tumoranreicherung ist die Sensitivitaet der 123J-E2-Szintigraphie beim Tumornachweis in der Primaerdiagnostik und der Lokalisation von Rezidiven und Metastasen im Vergleich zu gering. Eine sichere Differenzierung zwischen malignen und benignen Laesionen erscheint nicht ohne weiteres moeglich. Die 123J-Szintigraphie ist ein bildgebendes In-vivo-Verfahren zur Erkennung maligner Mamatumoren und laesst Aussagen ueber die Tumorlokalisation zu. Sie koennte eine spezifische Methode zur nichtinvasiven In-vivo-Diagnostik des ER-Status darstellen und bietet daneben neue Perspektiven in der Nachsorge des Mammakarzinoms. Als rezeptorspezifisches bildgebendes Verfahren hat die E2-Szintigraphie moeglicherweise bei Fragestellungen nach der Tumor-Heterogenitaet oder nach einem Wechsel des Rezeptorstatus unter Therapie eine Anwendungsberechtigung.
