Influence of hypoxia on the internalization and radioenhancement effect of bismuth-platinum nanoparticles in tumor cells treated with photon and fast ions

External radiotherapy plays a central role in the treatment arsenal of more than half of all cancer patients. However, its use is restricted by the side-effects on healthy tissue and the radioresistance often present in tumors. It is therefore crucial to explore alternative therapeutic methods to overcome these limitations. The combination of nanoagents with external radiotherapy is proposed as a strategy to increase the effect of radiation on the tumor, with the aim of achieving the same therapeutic effect with a lower total dose delivered to the patient. As these nanoagents are detectable by medical imaging, their concentration in the tumor after injection can be determined and diagnosis improved. These agents are known as 'theranostic'. When imaging and treatment are combined, we speak of 'image-guided therapy'. The combination with heavy ion beams offers promising prospects, combining the biological efficiency of ions, superior to that of X-rays, with the nanoagents. The aim of my thesis was to quantify, in vitro, the efficacy of new bimetallic nanoparticles composed of bismuth and platinum, coated with PEG diamine. After optimizing the synthesis of these agents, I assessed their toxicity and internalization capacity in cancer cell lines. Their radio-amplifying properties were evaluated in association with different types of photonic and hadronic radiations. The effect of molecular oxygen in the environment was also studied. Results showed that these nanoparticles, about 40 nm in diameter, can be functionalized with a fluorophore, rhodamine. They are stable, with low toxicity down to a concentration of 0.25 mM, and localize in the cytoplasm of cells, either in free form or concentrated in vesicles. Experiments on a 3D cell model have shown that nanoparticles are able to migrate into the extracellular matrix. Oxygen concentration had little influence on internalization. This work has also shown that these nanoparticles amplify the effect of photonic and hadronic ionizing radiation. Under normoxic conditions, they increase the effectiveness of X-rays (200 kV) by 62% at a dose of 2 Gy in HeLa cells, corresponding to a dose reduction by a factor of 1.58 (DEF) with a cell survival rate of 10%. At a dose of 2 Gy, nanoparticles also increase the efficacy of gamma radiation from a Cesium 137 source (13%) and from carbon (24%) and helium (38%) ion beams. The concentration of oxygen in the environment has a strong influence on the radiation-amplifying effect: the effect is annihilated in anoxic conditions (0% oxygen). This effect is attributed to oxygen's role in fixing molecular damage. The various stages involved in characterizing nanoparticles and their properties are essential for the development of new agents to be combined with different irradiation modalities for clinical application. Artificial intelligence could, in the future, be a tool to help choose the best combined treatment strategy. (author)

La radiotherapie externe joue un role central dans l'arsenal de traitements pour plus de la moitie des patients atteints de cancer. Cependant, son utilisation est restreinte en raison des effets secondaires qu'elle peut provoquer sur les tissus sains et de la radioresistance presente au sein de certaines tumeurs. Il est donc crucial d'explorer des methodes therapeutiques alternatives permettant de repousser ces limites. L'association de nanoagents a la radiotherapie externe est proposee comme strategie pour augmenter l'effet du rayonnement sur la tumeur en vue d'obtenir un meme effet therapeutique avec une dose totale delivree au patient plus faible. Ces nanoagents etant detectables par imagerie medicale, leur concentration dans la tumeur apres injection peut etre determinee et le diagnostic ameliore. Ces agents sont dits 'theranostiques'. Lorsque l'imagerie et le traitement sont concomitants, on parle de 'therapie guidee par imagerie'. La combinaison avec des faisceaux d'ions lourds offre des perspectives prometteuses qui associent l'efficacite biologique des ions, superieure a celle des rayons X, a celle des nanoagents. Ma these avait pour but de quantifier, in vitro, l'efficacite de nouvelles nanoparticules bimetalliques composees de bismuth et de platine, recouvertes de PEG diamine. Apres avoir optimise la synthese de ces agents, j'ai evalue leur toxicite et leur capacite d'internalisation dans des lignees cellulaires cancereuses. Leur propriete radioamplificatrice a ete evaluee dans le cas de leur association avec differents types de rayonnements photoniques et hadroniques. L'effet de l'oxygene moleculaire dans l'environnement a egalement ete etudie. Les resultats obtenus ont montre que ces nanoparticules, d'un diametre d'environ 40 nm, peuvent etre fonctionnalisees avec un fluorophore, la rhodamine. Elles sont stables, peu toxiques jusqu'a une concentration de 0,25 mM et se localisent dans le cytoplasme des cellules, sous forme libre ou concentree dans des vesicules. Des experiences sur un modele cellulaire 3D ont permis de montrer que les nanoparticules sont capables de migrer dans la matrice extra-cellulaire. La concentration d'oxygene influence peu l'internalisation. Ces travaux ont egalement permis de montrer que ces nanoparticules amplifient l'effet des rayonnements ionisants photoniques et hadroniques. En condition normoxique, elles augmentent l'efficacite des rayons X (200 kV) de 62% a une dose de 2 Gy dans des cellules HeLa, ce qui correspond a une diminution de la dose d'un facteur de 1,58 (DEF) avec un taux de survie cellulaire de 10%. a une dose de 2 Gy, les nanoparticules augmentent egalement l'efficacite du rayonnement gamma issus d'une source de Cesium 137 (13%) et des faisceaux d'ions carbone (24%) et d'ions helium (38%). La concentration en oxygene dans l'environnement influence fortement l'effet radio-amplificateur: l'effet est annule en condition anoxique (0% d'oxygene). Cet effet est attribue au role de l'oxygene dans la fixation des dommages moleculaires. Les differentes etapes de caracterisation des nanoparticules et de leurs proprietes sont indispensables au developpement de nouveaux agents a combiner avec differentes modalites d'irradiation en vue d'une application clinique. L'intelligence artificielle pourra, dans le futur, etre un outil permettant d'aider au choix de la meilleure strategie de traitement combine. (auteur)
