Striving for accident avoidance at Swedish plants

During refuelling outages at nuclear power plants, deficiencies can be corrected but potential failure mechanisms can be introduced if work is of low quality. During operation, major failure can be prevented if minor deviations are picked up at an early stage, eg using leak detection, mass balance and noise detection. The attitude of management and the training of personnel are also of key importance in maintaining plant safety. (author)
