Assessment of operational risk on the development seismic system of the RSG-GAS reactor

Description

The seismic monitoring system contained in the vertical panel of the Main Control Room of the Multipurpose Reactor - G.A. Siwabessy functions to provide warnings of earthquake events to the reactor supervisor. Decision making with a lack of specific indicators will result in several negative impacts based on human error. This is based on the absence of instrumentation devices that can display the value of ground acceleration in real time. The seismic instrumentation system in the RSG-GAS reactor is in the form of a light indicator that divides the earthquake alarm into two levels, namely the ″seismic recording″ alarm (level 1) and the ″earthquake″ alarm (level 2). Both of them cannot stop the reactor operation automatically, but they function to give a warning to the Supervisor to make a decision to continue or stop the reactor operation. Thus, a study (including a risk assessment) is needed for the development of a more technically useful seismic instrumentation system. This risk assessment identifies potential barriers which include the introduction of activities and the introduction of potential barriers. Conduct a risk assessment according to the scope, nature, and time to ensure that it is proactive and not reactive; and identification of priorities, and application of appropriate controls. From the operational risk assessment carried out, the highest risk rating ratings are 16 namely with the potential barriers to decision-making when the evacuation is disrupted due to the absence of appropriate information and 15, namely with the potential obstacles to saving the SSK reactor, especially related to environmental safety, experiencing delays. This means that the risk assessment rating has a 'D' predicate with the risk not yet acceptable, so that additional priority control actions need to be carried out. (author)
