BMIPP uptake in relation to PET imaging

Description
While in-vivo myocardial metabolic studies have been most intensively studied with PET, iodinated fatty acid analogs have a great impact on metabolic imaging in vivo in many clinical centers due to widely availability of iodinated compounds supplied from the company. An iodinated branched fatty acid analog, BMIPP provided excellent images of the left ventricular myocardium and may probe myocardial energy metabolism in vivo. The segments with discordant BMIPP uptake less than thallium which was often seen in patients with coronary artery disease may represent ischemic but viable myocardium where increase in glucose metabolism is also observed. In addition, oxidative metabolism is also preserved in such myocardium. Therefore, the combined imaging using BMIPP and thallium permits detection of ischemic but viable myocardium on the basis of alteration of myocardial energy metabolism. (orig.)
Abstract (German)
Obwohl der myokardiale Metabolismus in vivo am intensivsten mit PET untersucht wurde, haben radioiodierte Fettsaeureanaloga in vielen klinischen Zentren eine grosse Bedeutung in der in vivo bildgebenden Darstellung des Stoffwechsels, begruendet in der breiten kommerziellen Verfuegbarkeit der iodierten Pharmaka durch die Firmen. Das verzweigte iodierte Fettsaeureanalogon BMIPP liefert hervorragende Bilder des linksventrikulaeren Myokards und koennte als In-vivo-Substrat des myokardialen Energiestoffwechsel dienen. Segmente mit diskordanter verringerter BMIPP-Aufnahme im Vergleich zum Thallium, was haeufig bei Patienten mit koronarer Herzkrankheit beobachtet wurde, koennten ischaemisch aber vitales Myokard repraesentieren, in dem ebenso ein erhoehter Glukosestoffwechsel nachgewiesen wird. Auch ist der oxidative Stoffwechsel in diesen Myokardanteilen enthalten. Daher erlaubt die kombinierte Bildgebung mit BMIPP und Thallium den Nachweis ischaemischen aber vitalen Myokards auf der Basis eines alterierten myokardialen Energiestoffwechsels.
