Nuclear heart function diagnostics. Studies during the first tracer passage and on the distribution equilibrium

Description

The author attempted to establish a method of evaluation for gated blood pool scintiscanning. The necessary hardware for this purpose was installed, and a method of evaluation for the image sequences was developed on the basis of the medicine-specific GAMMA-11 routines. In addition, the inflow phase directly after the injection was observed. Two different images were possible, i.e. left-oblique in 450 projection or right-oblique in 300 projection. The findings were evaluated by an analysis of the cyclic heart action and by a quantitative determination of global parameters of the heart function, e.g. ejection fraction, rate of volume change etc. The regional myocardial motility can be assessed after orthogonal transformation into an amplitude image and a phase image. A new type of transformation is presented, i.e. the ''quantized sine/cosine transformation'' which allows fast calculation without a floating-point processor. States of akinesia and dyskinesia are well imaged. The first-passage technique gives a general picture of the cardiopulmonary situation. The algorithms described were used in 26 cardiac examinations of 24 patients. Ten further patients were examined by angiography. Except for one case of hypokinesia of the back wall, all disturbances of motility were detected. The angiographic and scintigraphic ejection fractions are correlated with r = 0.98, the rates between first-pass and gated study with r = 0.92, and the rates between 12-picture and 48-picture sequences with r = 0.98. Abstract (German)

Ziel der vorliegenden Arbeit war die Einrichtung eines Auswertungsgangs fuer die EKG-getriggerte Herzbinnenraum-Szintigraphie. Dafuer wurden sowohl die ''Hardware''-Voraussetzungen geschaffen, als auch programmtechnisch unter Ausnutzung der medizinspezifischen GAMMA-11-Routinen ein Auswerteverfahren fuer die gewonnenen Bildsequenzen des menschlichen Herzens erstellt. Dabei wird zusaetzlich die Einstromphase unmittelbar nach Injektion verfolgt. Die Studien koennen in zwei verschiedenen Ansichten - linksschraeg in 45-Grad - oder rechtsschraeg in 30-Grad-Projektion - angefertigt werden. Die Auswertung der Studien geschieht einerseits durch Begutachtung der zyklisch abgespielten Herzaktion, andererseits durch quantitative Bestimmung globaler Herzfunktionsparameter wie Auswurfrate, Volumenaenderungsgeschwindigkeit u.a.. Die regionale Myokardmotilitaet wird mittels orthogonaler Transformation in ein Amplituden- und ein Phasenbild beurteilbar. Es konnte dabei eine neue Art, die ''quantisierte Sinus-Cosinus-Transformation'' vorgestellt werden. Sie gestattet eine Verrechnung in wenigen Minuten ohne Gleitkomma-Prozessor. Die Darstellung bringt A- und Dyskinesien deutlich zur Abbildung. Die Methode der ersten Tracer-Passage erlaubt eine Einordnung in die gesamte kardiopulmonale Situation. Die beschriebenen Algorithmen wurden bei 26 Herzuntersuchungen an 24 Patienten erprobt. Zehn Patienten waren angiografiert worden. Mit Ausnahme einer Hinterwandhypokinesie wurden saemtliche Motilitaetsstoerungen entdeckt. Die angiographisch und szintigraphisch festgestellten Auswurfraten korrelieren mit r = 0.98, die Raten zwischen First-Pass und Gated Study mit r = 0.92 und die Raten zwischen Zwoelfbild- und 48-Bild-Sequenz mit r = 0.98.
