Quantitative assessment of the arterial and portal components of liver perfusion during scintigraphy with sup(99m)Technetium sodium phytate

Quantitative assessment of the relative arterial and portal blood flows by calculation of the slopes of the arterial and venous component of the liver first-pass curve was performed in 67 patients with steatosis or cirrhosis referred for liver scintigraphy. In normal subjects, the portal venous blood flow amounts to 74.3 +- 3 % of total liver blood flow. In the present study, this figure was reduced to 58.6 +- 8.6 % in steatosis (n = 5) and 35.5 +- 14.6 % in cirrhosis (n=33). Analysis of liver blood flow enhances the sensitivity of radioisotopic scans in diffuse liver diseases and is of considerable value for the indication, prognosis and follow-up of surgical shunts
Abstract (French)
L'estimation quantitative de la contribution arterielle et portable au debit hepatique total est basee sur l'etude des pentes arterielle et veineuse de la courbe de fixation hepatique. Cette etude a ete effectuee chez 64 malades (steatoses ou cirrhoses) adresses pour scintigraphie hepatique. La circulation portale chez les sujets normaux est egale a 74,3 +- 3 % du debit hepatique total. L'etude a montre une reduction de la perfusion portale a 58,6 +- 8,6 % en cas de steatoses (n=5) et a 35,5 +- 14,6 % en cas de cirrhoses (n=33). Cette etude hemodynamique hepatique augmente la sensibilite diagnostique de la scintigraphie en cas d'atteinte hepatique diffuse.
