Malignant pulmonary tumors in children

Description

Pulmonary neoplasms in children are rare. The majority of tumors involving the respiratory system are metastatic and primary lung tumors are even more rare. Approximately 75% of primary lung tumors are malignant, the most frequent of which are adenoma, bronchogenic carcinoma, and pleuropulmonary blastoma. Due to the rarity of these malignancies, and the usually nonspecific clinical symptoms, they are often not considered in the differential diagnosis in children who present with persistent pneumonitis, coughing, and atelectasis. This often results in delayed definitive treatment and, in general, a worse prognosis. The role of the radiologist is that by using modern imaging techniques, to facilitate early recognition and staging, enabling adequate treatment and follow-up and thus, a better clinical prognosis. This article provides an overview of the most relevant entities of primary childhood pulmonary malignancies, with a particular emphasis on the imaging characteristics. (orig.)

Abstract (German)

Lungentumoren im Kindesalter sind selten; hauptsaechlich handelt es sich dabei um sekundaere Absiedelungen anderer Tumoren. Wesentlich seltener finden sich Primaertumoren der Lunge. Etwa 75% dieser Tumoren sind maligne; zu den haeufigsten Entitaeten zaehlen das Adenom, das Bronchuskarzinom und das pleuropulmonale Blastom. Auf Grund des seltenen Auftretens und der unspezifischen klinischen Symptomatik, mit denen sich die betroffenen Kinder praesentieren, werden diese Tumoren kaum in die differenzialdiagnostischen Ueberlegungen miteinbezogen. Das zoegert naturgemaess die korrekte Diagnosestellung, v. a. aber den Therapiebeginn hinaus und kann somit zu einer Verschlechterung der Prognose fuehren. Die Rolle der Radiologie liegt darin, mit der Nutzung der modernsten Schnittbildtechniken eine fruehzeitige Erkennung, ein umfassendes Staging und eine adaequate Therapiekontrolle zu ermoeglichen, um die Gesamtprognose der Kinder mit Lungentumoren zu verbessern. Im Folgenden soll ein Ueberblick ueber die wichtigsten Lungentumoren im Kindesalter gegeben werden. (orig.)
