Investigations into the myocardial metabolism of 123-I-ω-HDA in patients showing the clinical signs and symptoms of mitral valve prolapse and previously proven to have no coronary heart disease

Description

In 53 patients showing the clinical signs and symptoms of mitral valve prolapse (MVP) where coronary heart disease (CHD) had previously been excluded by angiography the myocardial metabolism of fatty acids was examined at rest using metabolic myocardial scintigraphy (MMS) and 123-I-ω-HDA as tracer substance. The results were correlated with those obtained by other methods of investigation (findings at rest and during exercise from ECG recordings, tests using a flow-guided catheter and radionuclide ventriculography, further from echocardiography as well as myocardial Tl redistribution scintiscanning during exercise). For purposes of reference, MMS was additionally carried out in twelve normal individuals subjected to coronary angiography to exclude both MVP and CHD. The results of this study would appear to suggest that the pathological findings revealed by myocardial Tl scintigraphy in patients showing symptoms of MVP cannot invariably be classified as 'false' but are at least to some extent attributable to impaired myocardial perfusion, vitality or metabolism. Abstract (German)

Bei 53 symptomatischen Patienten mit Mitralklappenprolaps (MVP) und angiographisch ausgeschlossener koronarer Herzkrankheit (KHK) wurde eine metabolische Myokardszintigraphie (MMS) mit 123J-ω-Heptadekansaeure (HDA) in Ruhe zur Untersuchung des myokardialen Fettsaeurestoffwechsels durchgefuehrt. Die Ergebnisse wurden mit denen anderer Untersuchungen (Ruhe- und Belastungs-EKG, Einschwemmkatheter in Ruhe und unter Belastung, Echokardiographie, Radionuklidventrikulographie in Ruhe und unter Belastung und Belastungs-Redistributions-Tl-Myokardszintigraphie) korreliert. Zusaetzlich wurde bei zwoelf koronarangiographierten Patienten ohne MVP und ohne KHK eine MMS zur Kontrolle des Normalbereiches vorgenommen. Nach den Ergebnissen dieser Arbeit sind die pathologischen Befunde bei der Tl-Myokardszintigraphie bei symptomatischen Patienten mit MVP nicht saemtlich 'falsch pathologisch', sondern beruhen zumindest zum Teil auf Stoerungen der myokardialen Perfusion oder Vitalitaet oder des myokardialen Stoffwechsels.
