Development of a procedure for qualitative and quantitative evaluation of human factors as a part of probabilistic safety assessments of nuclear power plants. Part B. Technical documentation

Description

As international studies have shown, accidents in plants are increasingly caused by combinations of technical failures and human errors. Therefore careful investigations of man-machine-interactions to determine human reliability are gaining importance worldwide. Regarding nuclear power plants such investigations are usually carried out within the scope of probabilistic safety assessments. A great number of procedures to evaluate human factors has been developed up to now. However, none of them is able to take into account the whole spectrum of requirements - as for instance transferability of date to other plants, analysis of weak points, and evaluation of cognitive tasks - for a complete and reliable probabilistic safety assessment. Based on an advanced model for a man-machine-system, the Human Error Rate Assessment and Optimizing System (HEROS) and a corresponding expert system of the same name are introduced. This expert system enables the quantification of human error probabilities for plant operator actions on the one hand and is also capable of providing quantitative statements regarding the optimization of man-machine-system in terms of human error probability minimization on the other one. Three relevant evaluation levels, i.e. 'Management Structure', 'Working Environment' and 'Man-Machine-Interface', are derived from a model of the man-machine-system. Linguistic variables are assigned to all performance shaping factors at these levels. These variables are used to establish a rule-based expert system. The knowledge bases of this system are represented by rules. Processing of these rules is carried out by means of the fuzzy set theory, after provision of relevant data for a particular personal action to be evaluated. This procedure enables a simple and effective use of ergonomic studies as the relevant database, which is also transferable to other plants with any design. The expert system consist in total of 16 rule bases in which all ascertainable and assignable performance shaping factors are gathered. However, regarding the evaluation of cognitive tasks, criteria are derived from a psychologic pattern which are included in a corresponding rule base. First Calculations for the validation of the procedure are performed based on three examples, reflecting the common practice of probabilistic safety assessments on the one hand and including problems, which cannot be evaluated or can hardly be evaluated with the established human risk analysis procedures on the other one. The results of the HEROS application are plausible, comprehensible and can be compared with results of established procedures. Basic approaches for optimizing the man-machine-system may be also obtained from a sensitivity studies using HEROS. The new procedure may be further developed to be applied to other technical scopes and allow further developments, particularly regarding the 'learning' of data using the NeuroFuzzy-Technology. Internationale Studien belegen, dass Unfaelle in technischen Anlagen zunehmend auf Kombinationen von technischem und menschlichem Versagen zurueckzufuehren sind. Daher wird der Untersuchung der Zuverlaessigkeit von Mensch-Maschine-Wechselwirkungen weltweit grosse Beachtung beigemessen. Fuer Kernkraftwerke werden solche Untersuchungen im Rahmen von probabilistischen Sicherheitsanalysen durchgefuehrt. Hierzu existieren zwar eine Vielzahl von Verfahren, jedoch wird keines allen als wichtig erachteten Anforderungen - wie Uebertragbarkeit von Daten auf andere Anlagen, Schwachstellenanalyse und Bewertung kognitiver Handlungen - vollstaendig gerecht. Aufbauend auf einem modernen Modell eines Mensch-Maschine-Systems werden das Human Error Rate Assessment und Optimizing System (HEROS) und ein gleichnamiges Expertensystem vorgestellt, mit dem einerseits die menschliche Fehlerwahrscheinlichkeit fuer Personalhandlungen quantifiziert werden kann und andererseits auch qualitative Aussagen zur Optimierung des Mensch-Maschine-Systems im Hinblick auf eine Minimierung der Fehlerwahrscheinlichkeit moeglich sind. Hierzu werden aus dem Modell eines Mensch-Maschine-Systems zunaechst die drei Bewertungsbereiche Organisationsstruktur, Arbeitsumgebung und Mensch-Maschine-Schnittstelle abgeleitet. Innerhalb dieser Bewertungsbereiche werden fuer alle leistungsbeeinflussenden Faktoren linguistische Variable aufgestellt, welche der Erstellung eines unscharfen regelbasierten Expertensystems dienen. Die Wissensbasis des Expertensystems wird dabei durch Regeln repraesentiert. Ihre Verarbeitung erfolgt nach der Bereitstellung von Daten fuer die zu bewertende Personalhandlung unter Nutzung der Fuzzy Set Theorie. Diese ermoeglicht die einfache und effektive Nutzung allgemeingueltiger arbeitswissenschaftlicher und ergonomischer Studien als Datenbasis, die auf Anlagen beliebigen Designs uebertragbar ist. Das Expertensystem besteht aus insgesamt 16 einzelnen Regelbasen, mit denen alle ermittel- und bestimmbaren Einflussfaktoren erfasst sind. Auch fuer kognitive Handlungen werden aus einem psychologischem Modell Bewertungskriterien abgeleitet, die deren Analyse mit einer entsprechenden Regelbasis ermoeglichen. Erste Rechnungen zur Validierung des Verfahrens erfolgen anhand von drei Beispielen, die einerseits die haeufige Praxis probabilistischer Sicherheitsanalysen widerspiegeln und andererseits auch Probleme beinhalten, die mit etablierten Verfahren nicht oder nur sehr schlecht erfassbar sind. Diese Anwendungen von HEROS zeigen plausible und nachvollziehbare Ergebnisse, die auch mit etablierten Verfahren vergleichbar sind und die Moeglichkeit der direkten Ableitung von Optimierungsansaetze fuer das Mensch-Maschine-System durch eine Sensitivitaetsanlyse bieten. Das Verfahren ist auf andere Einsatzgebiete erweiterbar und bietet Entwicklungsmoeglichkeiten bis hin zum 'Erlernen' von Daten mittels der NeuroFuzzy-Technologie. (orig.)
