Prognosis contribution of PET-CT with [18F]-FDG for stage II bronchial cancers without small cells
Description
The first objective of this medicine thesis has been to assess the prognosis contribution of PET-Computerised Densitometry in the case of stage II bronchial cancers without small cells. The author first gives an overview of bronchial carcinogenesis (morphological anomalies, neoplastic character and lesion evolution, molecular anomalies involved in lung carcinogenesis), proposes an anatomic-pathological classification of bronchial-lung cancers. He reports and comments epidemiological data related to these cancers in France (recent data, risk factors, screening), and presents the TNM classification and the way it is used for bronchial cancer diagnosis. He describes the different approaches to take these cancers into care (surgery, chemotherapy, radiotherapy, vaccine-therapy). He discusses prognosis factors (clinical, biological, histopathological, and molecular factors), presents the positron emission tomography (PET) technique, notably with fluorodeoxyglucose (FDG), and finally reports and comments actual results obtained by patient treatment
Abstract (French)
La TEP-FDG est devenu une etape obligatoire du bilan preoperatoire des cancers bronchiques non a petites cellules. Les patients de stade II beneficient a l'heure actuelle, en l'absence de contre-indication, d'une chirurgie suivie d'une chimiotherapie adjuvante. Le benefice de cette derniere en termes de survie globale est de 5% mais il y a actuellement trop peu de facteurs pronostiques utilisables dans les cancers du poumon. Le TEP-FDG au travers du SUV apparait comme un facteur pronostique potentiel qui manque cependant de reproductibilite inter-centre. Methode: Etude retrospective sur 59 patients de 2005 a 2009 a l'hopital de Limoges et de Tenon. Nous avons evalue l'apport pronostique en termes de survie sans recidive et de survie globale du ratio SUVmax (defini par le rapport entre le SUVmax tumoral et le SUVmax hepatique), du ratio SUVmean 50% (defini par le rapport entre SUVmean 50% tumoral et le SUVmax hepatique) ainsi que du volume tumoral metabolique. Resultats: L'analyse univariee a montre que le ratio SUVmax (p=0,0171), le ratio SUVmean50% (p=0,0108), l'envahissement ganglionnaire (p=0,0053) et le stade TNM (p=0,0136) etaient predictifs de recidive. Lors de l'anayse de survie globale, le ratio SUVmax (p=0,0139), le ratio SUVmean50% (p=0,0072), le volume tumoral metabolique (p=0,030), l'envahissement ganglionnaire (p=0,0139), l'anemie inferieure a 12g/d (p=0,0497) et le performance status (p=0,0437) apparaissent predictifs en analyse univariee. En analyse multivariee, seul l'envahissement ganglionnaire est un facteur independant que ce soit en termes de survie globale ou sans recidive. Le SUV est significativement associe au Ki67 et a l'histologie.Conclusion: Le ratio SUV max et le ratio SUVmean 50% sont des donnees reproductibles entre differents centres et devraient etre pris en compte dans la decision de chimiotherapie adjuvante. L'envahissement ganglionnaire reste le facteur le plus important de notre etude. (auteur)Files
