Surgical management of lymph node metastases from papillary thyroid carcinoma
Description
The published evidence and discussion of the various approaches to papillary thyroid carcinoma are reviewed. The impact of lymph node metastases on oncological results, the role of ultrasound, the central compartment approach, the lateral compartment approach, central neck dissection, lateral neck compartment dissection, complications such as hypoparathyroidism, nerve injury Recurrent larynx, thoracic duct injury, bleeding, facial and brain edema, rupture of the internal carotid artery, and management of recurrences were addressed and discussed. Customizing each decision according to the risk of local recurrence was recommended. Dissection of the central compartment with therapeutic indication was justified based on the evidence. The evidence-based debate on the prophylactic indication for central compartment dissection continues due to conflicting results. Modified radical dissection or selective dissections of lateral compartments were indicated as therapeutic use since they have solid scientific bases. The prophylactic indication for dissections of the lateral compartments of the neck lacks support in the published literature. Surgery was indicated as the treatment of choice for regional recurrences both centrally and laterally in order to achieve optimal cure rates

La evidencia publicada y la discusion de los diversos abordajes para el carcinoma papilar de tiroides son revisadas. El impacto de las metastasis ganglionares en los resultados oncologicos, el rol del ultrasonido, el abordaje del compartimento central, el abordaje del compartimento lateral, la diseccion central del cuello, la diseccion de compartimentos laterales del cuello, complicaciones como el hipoparatiroidismo, lesion de nervio laringeo recurrente, lesion del conducto toracico, sangrado, edema facial y cerebral, ruptura de la arteria carotida interna y manejo de las recurrencias fueron abordados y discutidos. Personalizar cada decision segun el riesgo de recurrencia local fue recomendado. La diseccion del compartimento central con indicacion terapeutica fue justificada basada en la evidencia. El debate sobre la indicacion profilactica de la diseccion de compartimento central basado en la evidencia continua debido a los resultados contradictorios. La diseccion radical modificada o las disecciones selectivas de compartimentos laterales fueron indicadas como uso terapeutico ya que cuentan con bases cientificas solidas. La indicacion profilactica de las disecciones de los compartientos laterales del cuello carece de sustento en la literatura publicada.
