Hyperfractionated irradiation plus Cisplatin for treatment of advanced head and neck cancer

Description
From January 1988 to April 1989, 22 of 28 patients with advanced carcinomata of the head and neck have been treated by hyperfractionated radiotherapy (2x1.2 Gy/day, five days/week, total dose 74.4 to 79.2 Gy) and simultaneous application of Cisplatinum (20 mg/m2 day, i.v. five days in the first and fifth week of the radiotherapy). In 68% of the patients the treatment could be applied in full dose. All eight patients with primary application of a gastrostomy could be irradiated without split, whereas five of 14 patients without a gastrostomy had a split of ≥2 weeks. The mucositis healed in all patients after the end of the irradiation. Up to now, no serious complications have been seen. One year cumulative locoregional control rate is 65%. This study serves as a pilot study for a randomized trial. According to the acute and subacute toxicity the applied treatment is feasible. (orig.)
Abstract (German)
Als Vorbereitung fuer eine randomisierte Studie wurden von Januar 1988 bis April 1989 22 von 28 zugewiesenen Patienten mit fortgeschrittenen Karzinomen des ORL-Bereiches mit hyperfraktionierter Radiotherapie (2 mal 1,2 Gy/Tag, fuenf Tage/Woche, 74,4 bis 79,2 Gy Gesamtdosis) und simultaner Cisplatinum-Gabe behandelt (20 mg/m2 i.v. eine Stunde vor der zweiten Bestrahlung an fuenf Tagen in der ersten und fuenften Bestrahlungswoche). In 68% der Patienten konnte die geplante Dosis des CDDP und der Radiotherapie appliziert werden. Alle acht Patienten mit primaerer Applikation einer Gastrostomie (perkutane endoskopische Gastrostomie) wurden ohne Pause bis zur Enddosis bestrahlt, waehrend bei fuenf von 14 Patienten ohne Gastrostomie eine Pause von ≥14 Tagen erforderlich war. Die Mukositis ist in allen Faellen ohne Probleme nach Ende der Radiotherapie abgeheilt. Schwerwiegende Komplikationen sind bisher nicht aufgetreten. Die kumulative lokoregionale Kontrollrate ueber ein Jahr betraegt 65%. Hinsichtlich der akuten und subakuten Toleranz ist das applizierte Behandlungsschema praktikabel. (orig.)Additional details
