Sequels after radiotherapy of adenoma of adenohypophysis

14 years before his death, iridium seeds were implanted in the adenohypophysis of a 69-year-old patient with a STH producing adenoma of the adenohypophysis and acromegaly. 5 years after recurrence, transsphenoidal hypophysectomy was done. The patient died of central dysregulation due to cerebral insults. At autopsy, a recurrence of the undifferentiated acidophilic adenoma was found within the sphenoidal bone corpus. Immunohistochemistry was positive for STH, prolactin, TSH, LH, and FSH. Besides the adenoma, a mucoepidermoid carcinoma was found within the sphenoid cavity. This carcinoma may be a consequence of the radiotherapy of the adenoma of the adenohypophysis. (orig.)

Abstract (German)

Bei einem 69 Jahre alt gewordenen Patienten mit Akromegalie war 14 Jahre vor dem Tod eine Radio-Iridium-Einlage in ein STH-prodzierendes Hypophysenadenom implantiert worden. Wegen Rezidiv wurde 5 Jahre spaeter eine transsphenoidale Hypophysektomie durchgefuehrt. Der Tod erfolgte durch eine zentrale Dysregulation bei zusaetzlich bestehenden Hirninfarkten. Bei der Obduktion fand sich bei vernarbter leerer Sella ein Rezidiv des undifferenzierten azidophilen Hypophysenadenoms im Keilbeinkoerper, in welchen immunhistochemisch STH und in geringen Mengen Prolaktin, TSH, LH und FSH nachweisbar waren. Daneben wurde ueberraschend ein mukoepidermoides Karzinom gefunden, das offenbar mit der vorangegangen Bestrahlung in Zusammenhang stand und von der Keilbeinhoehle ausgegangen war. (orig.)
