Bone scintigraphy with sup(99m)Tc-pyrophosphate pharmacokinetics and clinical experience in scintigraphy

Kinetics and clinical studies of bone scintiscanning with sup(99m)Tc-pyrophosphate in 92 patients with certified diagnosis allowed the following conclusions: After i.v. injection sup(99m)Tc-pyrophosphate is distributed in 3 compartments, from which it is cleared with biological half-lives of 32.7 minutes, 5.1. hours and 22.54 hours respectively. The relative size of these compartments is 1 : 3 : 6. After application of the substance, an initially high renal excretion with a consecutive exponential drop can be observed. The optimal period for scintigraphic studies seems therefore to be from 3-6 hrs, as at this time there is still an adequate activity in bone structures, while the low background activity yields a good contrast on scintiscans. The clinical results of bone scintigraphy with sup(99m)Tc-pyrophosphate with 88.6% correct diagnoses can be considered as 'very good'. This feature can partly be attributed to an improved correlation of radiographic and scintigraphic images by the use of a specially designed, combined radiographic-photoscanning detector system. Preliminary results concerning the quantitative uptake of sup(99m)Tc-pyrophosphate in fractures of the extremities suggest, that the quantitative analyses of the registered data can provide valuable prognostic information in traumatic, inflammatory and neoplastic bone lesions. Abstract (German)

Kinetische und klinische Untersuchungen an insgesamt 92 Patienten mit 99mTc-Pyrophosphat zur Skelettszintigraphie bei verifizierten Diagnosen der Probanden ergaben folgende Schlußfolgerungen: 1. Nach i.v. Injektion verteilt sich 99mTc-Pyrophosphat in 3 Kompartments, aus welchen es mit biologischen Halbwertszeiten von 32,7 Minuten respektive 5,1 Stunden bzw. 22,54 Stunden eliminiert wird. Die relative Größe dieser Kompartments verhält sich wie 1: 3: 6. Nach Applikation der Substanz erfolgt eine initial hohe Ausscheidung aus dem Organismus über die Nieren, welche in der Folge exponentiell abfällt. Der optimale Zeitpunkt für die Szintigraphie des Skelettsystems scheint daher zwischen 3 und 6 Stunden zu liegen, da zu diesem Zeitpunkt einerseits eine ausreichend hohe Aktivität im Skelett erwartet werden kann, während andererseits kontrastreiche Szintigramme ohne störende Untergrundaktivität geschrieben werden können. 2. Die klinische Treffsicherheit der Skelettszintigraphie mit 99mTc-Pyrophosphat ist mit 88,6% richtiger Diagnosen als sehr gut zu bezeichnen, wobei dieser Wert zum Teil wohl durch die verbesserte Zuordnung von Szintigrammbefund zu Röntgenbefund durch Verwendung eines entsprechend ausgelegten, kombinierten Röntgen-Szintigraphie-Arbeitsplatzes erzielt werden konnte. 3. Erste Ergebnisse über die quantitative Aufnahme von 99mTc-Pyrophosphat in Extremitätenfrakturen während der Frakturheilung lassen vermuten, daß sich die quantitative Analyse der Meßergebnisse auch prognostisch für Verlaufsbeobachtungen bei Frakturen, sowie bei entzündlichen und neoplastischen Knochenerkrankungen einsetzen läßt.
