X-ray contrast media - pathophysiological causes of their side effects

X-ray contrast media are chemicals used for diagnosing which have toxicity factors due to their clinical and physical properties. These toxicity factors can have both local effects and affect the whole system. Besides the dose-dependent changes in various organs we have also observed side effects which obviously did not depend on the dose. Side effects not depending on the dose are caused by mediator substances. There are various divergent opinions on the release of mediator substances. Unspecific, non-immunogenic processes are described to appear in addition to the ''classical'' allergy to the contrast medium molecule or its components. (orig.)

Abstract (German)

Roentgenkontrastmittel sind Chemodiagnostika, die aufgrund ihrer klinischen und physikalischen Eigenschaften Toxizitaetsfaktoren aufweisen. Diese koennen sich lokal wie systemisch auswirken. Neben dosisabhaengigen Veraenderungen an den verschiedenen Organen werden auch offensichtlich dosisunabhaengige Nebenwirkungen beobachtet. Dosisunabhaengige Nebenwirkungen werden durch Mediatorstoffe hervorgerufen, ueber die Ursachen der Mediatorstofffreisetzung bestehen im Schrifttum kontroverse Ansichten. Neben ''klassischen'' Allergien gegen das Kontrastmittelmolekuel oder seine Bestandteile, werden unspezifische nicht immunogene Vorgaenge beschrieben. (orig.)
