The idiopathic varicocele of the testis
Description
Retrograde sclerotherapy is the treatment of choice with the highest number of published case reports. As compared to surgical treatment, sclerotherapy is much more successful, does not run the risk of involving post-treatment complications such as hydrocele, and is much less invasive at that. Sclerotherapy requires only short time for diagnostic examination and after treatment, the out-patients recover very quickly and regain working capacity, depending on the physical requirements at their place of work. A competing method is antegrade sclerotherapy, which involves higher radiation doses, although the assessment by Boeck, published as early as in 1984, shows that the radiation exposure is fairly low. However, retrograde sclerotherapy is less invasive as there is no need to cut the scrotal skin. This is an aspect to be considered primarily in the treatment of children and juveniles. (orig./CB)
Abstract (German)
Als Therapie der Wahl mit den meisten publizierten Fallzahlen gilt die retrograde Sklerosierung. Gegenueber den chirurgischen Verfahren hat sie teilweise erheblich hoehere Erfolgsraten, es droht im Gegensatz zur chirurgischen Therapie nicht die Komplikation einer Hydrocele und die Methode ist deutlich weniger invasiv als alle chirurgischen Verfahren. Bei der retrograden Sklerosierung ergibt sich eine kurze Untersuchungszeit und nach der ambulanten Behandlung ist der Patient, je nach beruflicher koerperlicher Belastung, bereits nach wenigen Tagen wieder arbeitsfaehig. Ein Konkurrenzverfahren stellt die antegrade Sklerosierungstherapie dar. Bei dieser ist zwar die Strahlenbelastung hoeher, wobei bereits Boeck 1084 nachwies, dass die Werte relativ niedrig liegen, die retrograde Sklerosierung ist jedoch andererseits noch weniger invasiv, da kein Scrotalhautschnitt notwendig ist. Dieser Aspekt spielt gerade bei der Behandlung von Kindern und Jugendlichen eine nicht zu unterschaetzende Rolle. (orig./ MG)Additional details
