Brain perfusion scintigraphy in diagnosis of brain stem ischemic stroke: A case report

Description

Brain stem ischemic stroke are characterized by nonspecific clinical manifestations. Another problem is the usual normality of cerebral CT scan. Cerebral magnetic resonance imaging is the gold standard for diagnosis. However, this exploration can be not available or contraindicated in many situations. Brain perfusion scintigraphy can be a diagnostic option. It allows objectifying interruption or significant impairment of blood supply to the brain and detecting both the presence and the extent of infarction. We report the case of a patient who is admitted in intensive care for coma in whom the diagnosis of brain stem ischemic stroke is confirmed by brain perfusion scintigraphy. (authors)

Abstract (French)

Les accidents vasculaires ischemiques du tronc cerebral sont caracterises par des manifestations cliniques aspecifiques. Une autre difficulte diagnostique est l'habituelle normalite de la tomodensitometrie. L'imagerie par resonance magnetique est l'examen de reference par excellence. Cependant, cette derniere exploration peut etre non disponible ou contre-indiquee dans plusieurs situations. La scintigraphie cerebrale de perfusion peut etre une alternative diagnostique. Elle permet d'objectiver la diminution ou l'interruption du debit sanguin cerebral et d'identifier la topographie et l'etendue du territoire ischemique. Nous rapportons le cas d'un patient admis en reanimation pour un coma et chez qui le diagnostic d'un accident vasculaire ischemique du tronc cerebral a ete confirme par la scintigraphie cerebrale de perfusion.
