Increased serum amylase in patients after radiotherapy as a probable bioindicator for radiation exposure

Description
The radiation-induced increase of serum amylase is investigated in 41 patients following either whole body irradiation or irradiation of the head- and neck-region. The radiation treatment caused a dose-independent serum amylase increase up to 80 times of the preirradiation measured in controls. This increase can only be induced if the salivary glands are within the radiation field. An isoenzyme analysis differentiated between salivary and pancreatic amylase. It was shown that also in case of whole body irradiation (wbi) pancreatic proteins either contribute only to a small extent to the increase or show no recognizable change. Although great variations in radiation response remain, the increase of serum amylase produced by salivary glands is highly significant and serves as a bioindicator for radiation exposures. (orig.)
Abstract (German)
Der strahleninduzierte Anstieg von Amylase im Serum wurde an 41 Strahlentherapie-Patienten untersucht, die entweder ganzkoerper- oder im Kopf-Nacken-Bereich teilkoerperbestrahlt wurden. Durch die Bestrahlung wurde ein von der Hoehe der Dosis unabhaengiger Anstieg der Serumamylase induziert, der bis zum 80fachen ueber den vor Bestrahlung gemessenen Kontrollwerten lag. Dieser Anstieg wird nur induziert, wenn die Speicheldruesen im Bestrahlungsfeld liegen. Mit einer Isoenzymanalyse wurde zwischen Speichel- und pankreatischer Amylase im Serum unterschieden. Dadurch konnte gezeigt werden, dass selbst im Falle von Ganzkoerperbestrahlungen der Anstieg der Serumamylase, wenn ueberhaupt, dann nur zu einem kleinen Teil von pankreatischer Amylase verursacht wird. Obwohl grosse Schwankungen im Anstieg der Serumamylase festgestellt wurden, so ist der Anstieg der Serumamylase aus den Speicheldruesen hoch signifikant und kann als Bioindikator fuer Expositionen mit ionisierender Strahlung dienen.
