Pneumonia in organization secondary to the use of everolimus: Case report

Objective: To report the case of a patient who developed organizing pneumonia (OP), presumably associated with the use of everolimus. Methodology: Study of the case report type, with data collected through medical records and literature review researched by PUBMED. Results: The case report is of a 66-year-old patient who underwent kidney transplantation and then started using tacrolimus and everolimus. One year and five months after the transplant, developed a dry cough and fever. Then she was admitted for investigation, and infectious and autoimmune causes were excluded. She progressed with clinical worsening, was intubated and remained in intensive care bed. Lung biopsy was performed with a result compatible with OP. The clinical picture was correlated with the use of everolimus, being suspended and prednisone started, evolving with clinical improvement. Conclusion: mTOR inhibitors are medications with a strong association with the development of interstitial lung diseases. The temporality, in the literature, varies from the onset of everolimus to the onset of the first symptoms. In this case, everolimus was the drug related to the development of OP, due to the largest number of cases in the literature when compared to tacrolimus, whose evidence is weaker. (author)
