Comparison of in-vivo kinetics of an antibody cocktail containing 131-iodine anti-CA-19/9 and 131-iodine anti-CEA with 111-indium labelled monoclonal anti-CA-19/9 using a tumor model in mice

In this study the potential diagnostic value of an 111-In-labelled CA-19/9-F (ab)-fragment was compared to that of an antibody cocktail of 131-iodine-labelled CA-19/9 and 131-iodine-labelled anti-CEA for identification of pancreas cancer by a nude mice model. 111-In-labelled CA-19/9 and the 131-iodine antibody cocktail were injected into 35 nude mice xenotransplantated with human pancreas cancer. Scintigrams were obtained and the relative distribution of activity in tumor and in several organs were determined by ROI-technique. These values were compared with the in vitro results of organ measurement after dissection of nude mice. Blood pool of 131-iodine-labelled antibodies showed only a nuclide accumulation in the thyroid because of very high rate of dejodination and missing blockade of thyroid. Other organs were not detectabel in scintigraphy because of high nucleotide accumulation of thyroid. The tumor-to-blood-ratio of organ-measurements was 18±4.3, kidneys-background-ratio 2.1±7.3, liver-background-ratio 5.8±2.0. These results are similar to those of 111-In-labelled fragments. Thus it is established that antibody cocktail had no essential advantage over singular antibody in mouse model. It gives a good tumor contrast with tumor-background-quotient of 15±7.4 measured by scintigraphy and tumor background-quotient 18±4.3 in-vitro-organ-measurement. (orig.)

Abstract (German)

In dieser Studie wurde anhand eines Mausmodells die diagnostische Wertigkeit eines 111-In-markiertes Anti-Ca-19/9-F(ab)-Fragmentes im Vergleich zu einem Antikoerpercocktail bestehend aus 131-J-markiertem Anti-Ca-19/9 und 131-J-markiertem Anti-CEA zur Identifizierung von Pankreastumoren untersucht. 35 Nacktmaeusen mit einem xenotransplantierten humanen Pankreastumor wurden simultan 111-In-markiertes Anti-Ca 19/9 und 131-J-markiertes Anti-Ca 19/9 und 131-J-markiertes Anti CEA injiziert. Es wurden Szintigramme erstellt und mittels ROI-Technik die relative Aktivitaetsverteilung im Tumor und in verschiedenen Organen bestimmt, und diese Werte mit den Ergebnissen der in-vitro-Organmessungen nach Sektion der Tiere verglichen. Wegen der hohen Dejodierungsrate konnten szintigraphisch nur die 111-In-Scans ausgewertet werden. Bei der Applikation der mit 131-Jod markierten inkompletten Antikoerper zeigte sich aufgrund der sehr hohen Dejodierungsrate und einer fehlenden Blockade der Schilddruese szintigraphisch fast ausschliesslich eine Nuklidakkumulation in der Schilddruese. Die uebrigen Organe stellten sich aufgrund der hohen Nuklidanreicherung in der Schilddruese szintigraphisch nicht dar. Die in-vitro-Organmessungen ergaben mit einem Tumor-Background-Quotienten von 18±4,3, einem Nierenbackground-Quotienten von 2,1±7,3 und einem Leber-Background-Quotienten von 5,8±2,0 aehnliche Werte wie bei den 111-Indium-markierten Fragmenten. Somit kann festgestellt werden, dass der Antikoerpercocktail im Vergleich zum einzelnen Antikoerperfragment im Mausmodell keinen wesentlichen Vorteil aufweist. Bei einem szintigraphisch gemessenen Tumor-Hintergrund-Quotienten von 15±7,4 und einem mittels in-vitro-Organmessung erhaltenen Tumor-Hintergrund-Quotienten von 18±4,3 ergibt sich ein guter Tumorkontrast.
