Background
Novel findings in molecular cancer research have not only improved the understanding of mechanisms driving formation of gliomas and other tumors of the central nervous system but also led to the identification of diagnostically relevant biomarkers for improved brain tumor classification.Objective
What is the current state of neuropathological glioma diagnostics according to the revised WHO classification of 2016? Which novel developments have further advanced glioma classification since then?
Material and methods
The integrated histological and molecular classification of gliomas according to the WHO 2016 classification is presented. New developments and perspectives of glioma diagnostics are discussed based on own experiences and the relevant literature since 2016.
Results
The WHO classification 2016 has led to significant refinements and improved reproducibility of the neuropathological diagnostics of gliomas. New developments in the past 2 years include the identification of promising novel biomarkers, the characterization of new entities and molecular subgroups of gliomas and the development of high-throughput methods for the determination of diagnostic mutation spectra and DNA methylation profiles.
Conclusion
The neuropathological diagnostics of gliomas are no longer based on histology alone but increasingly employ molecular diagnostic assessments, which fostered a markedly improved diagnostic precision as well as more accurate prediction of response to therapy and prognosis of glioma patients.

Zusammenfassung
Hintergrund
Neue Erkenntnisse der molekularen Tumorforschung haben nicht nur das Verständnis der Entstehungsmechanismen von Gliomen und anderen Tumoren des zentralen Nervensystems (ZNS) verbessert, sondern auch zur Identifizierung diagnostisch relevanter Biomarker für eine verbesserte Hirntumorklassifikation geführt.
Fragestellung
Wie ist der aktuelle Stand der neuropathologischen Diagnostik von Gliomen gemäß der überarbeiteten WHO-Klassifikation von 2016 und welche neuen Entwicklungen gibt es seither?
Material und Methode
Die integrierte histologische und molekulare Klassifikation von Gliomen gemäß der WHO-Klassifikation 2016 wird dargestellt. Darauf aufbauende neue Entwicklungen und Perspektiven der Gliomdiagnostik werden auf Basis eigener Erfahrungen und der relevanten Fachliteratur seit 2016 vorgestellt.
Ergebnisse
Die WHO-Klassifikation 2016 hat zu einer wesentlichen Präzisierung und verbesserten Reproduzierbarkeit der neuropathologischen Diagnostik von Gliomen geführt. Weiterentwicklungen der letzten beiden Jahre umfassen die Identifizierung neuer vielversprechender Biomarker, die Charakterisierung neuer Entitäten und molekularer Subgruppen von Gliomen sowie die Entwicklung von molekularen Hochdurchsatzmethoden zur Bestimmung von diagnostischen Mutationsspektren und DNA-Methylierungsprofilen.
Schlussfolgerungen
Die neuropathologische Diagnostik von Gliomen basiert neben der Histologie zunehmend auf molekularpathologischen Befunden, was zu einer erheblichen Verbesserung der diagnostischen Präzision sowie einer genaueren Vorhersage des Ansprechens auf Therapie und der Prognose von Patienten mit Gliomen geführt hat.
