Pretherapeutical diagnosis of metastases in carcinoma of the breast
Description
Conscentious pretherapeutic search for metastases is a perequisite for every curative local therapy of breast carcinoma. This must be preceded by thorough physical chekup, supplemented by the usual blood tests, thoraic x-ray, and contralateral mammography. The axillary lymphatic nodes should be biopsied, since clinical assessment alone, unsupported by other examination, yields false results in about on-third of the cases. Skeleton scintigraphy is recommended for routine skeleton diagnostics, possibly followed by specific roentgenography or tomography of individual skeleton sections. Bone marrow biopsy as well as individual biochemical examination (discussed in this article in detail) are hardly useful in the search for hidden skeleton metastases, but they can be of importance in assessing the course of therapy. If there is the liver, brain or other organs, additional special examinations must be performed. Scintigraphy in combination with laparoscopy or ultrasound tomography is recommended in liver diagnostics. Brain metastases can be reliably detected by means of computer tomography. (orig.) 891 MG
Abstract (German)
Eine gewissenhafte praetherapeutische Metastasensuche ist Voraussetzung jeder kurativ ausgerichteten lokalen Therapie des Mammkarzinoms. Am Anfang steht eine gruendliche koerperliche Durchuntersuchung, ergaenzt durch die ueblichen Blutuntersuchungen, Thoraxaufnahmen und eine Mammographie der Gegenseite. Die axiallaeren Lymphknoten sollten biopsiert werden, weil die klinische Beurteilung allein in etwa einem Drittel der Faelle falsche Resultate ergibt. Fuer die routinemaessige Skelettdiagnostik wird die Skelettszintigraphie empfohlen, eventuell gefolgt von gezielten Roentgenaufnahmen bzw. Tomogrammen einzelner Skelettabschnitte. Die Knochenmarksbiopsie sowie die im einzelnen angesprochenen biochemischen Untersuchen helfen bei der Suche nach okkulten Skelettmetastasen kaum weiter, sie koennen jedoch Bedeutung in der Verlaufsbeurteilung unter Therapie haben. Bei geringstem klinischen Verdacht auf Metastasen der Leber, des Gehirns oder anderer Oragne sind zusaetzliche Spezialuntersuchungen vorzunehmen. Fuer die Leberdiagnostik wird die Szintigraphie in Kombination mit der Laparoskopie oder Ultraschalltomographie empfohlen. Hirnmetastasen lassen sich zuverlaessig mit der computerisierten Tomographie nachweisen. (orig.)Additional details
