A probable radiological emergency in nuclear medicine

Full text: When a therapeutic dose of 131 I is indicated, especially in the thyroid carcinomas, the authorized doctor must always have present the possibility that the patient eliminates high activities of the radio-active material with the vomit. Keeping in mind that dose of 100 to 200 mCi is habitual in the carcinoma of thyroid, this episode can constitute a true radiological emergency, particularly because the first ones in taking knowledge of the fact can be people without appropriate preparation to this situation, what can cause contaminations difficult to manage them. Because it is not acceptable that a source open of high activity remains without treatment long time, the authorized doctor must act immediately, for that which should be prepared with anticipation, and have the necessary elements, to have an operative routine and to administer the storage of the polluted elements appropriately. To such an effect, we have orchestrated a sequential program of performance of 11 points, in the cases of plentiful vomits, with contamination of floors and things of the room. The program begins with the writing instructions for the patient and the personnel of infirmary in case of feeling desires to vomit, and de program is completed in case of being necessary. The elements are detailed in a handbag that contains for radiological emergencies for vomit. It notes that the low cost of the elements and clothes kind for surgery disposable. It discusses about the necessity of having prepared rooms for to receive patient with treatment with 131 I, in the clinics and public hospitals. (author)

Texto completo: Cuando se indica una dosis terapeutica de yodo 131, especialmente en los carcinomas tiroideos, el medico autorizado debe tener siempre presente la posibilidad que el paciente elimine altas actividades de material radiactivo con el vomito. Teniendo en cuenta que la administracion de actividades de 100 a 200 mCi son habituales en el carcinoma de tiroides, este episodio puede constituir una verdadera emergencia radiologica, particularmente porque los primeros en tomar conocimiento del hecho pueden ser personas sin preparacion adecuada para encarar la situacion, lo que puede ocasionar contaminaciones dificiles de manejar. Para controlar la situacion, el medico autorizado debe actuar de inmediato, para lo cual debe estar preparado con anticipacion, contar con los elementos necesarios, tener una rutina operativa y administrar adecuadamente el almacenamiento de los elementos contaminados. A tal efecto, hemos instrumentado un programa secuenciado de actuacion de 11 puntos, en los casos de vomitos copiosos, con contaminacion de piso y elementos de la habitacion. El programa se inicia con las instrucciones por escrito para el paciente y el personal de enfermeria, en casos de sentir deseos de vomitar y el programa se completa en caso de ser necesario. Se detallan los elementos que debe contener un maletin para emergencias radiologicas por vomito. Se destaca el bajo costo del mismo, integrado por elementos y ropa tipo cirugia descartables. Se discute la necesidad de contar con habitaciones praparadas para recibir pacientes con tratamiento con yodo 131, en clinicas y hospitales publicos. (autor)Files
