Electronic device for scintillation camera physiological driving

Description

The cardiac and breathing motions induce kinetic blurring on scintigraphic images. The realization of a device 'Pilotix' made of three probes taking the electrocardiographic and breathing signals is described. One can select the ends of systolic and diastolic stages or the respiratory pause phase, these lengths of signal appear on the scope as overbright. This device was connected with scintillation cameras. The clinical results show mainly an increasing resolution power on the organs outlines. Some equivocal interpretation on edge defects can be solved. In abdominal exploration the moving ability of one part or the whole organ with breathing motion can be appreciated. Images done during inspiratory and expiratory phases on the liver and the lungs demonstrate these organs deformation and translation very often associated with a rotation. This last kind of motion forbids all the type of corrections made on the only translation and the barycentric method which does an average correction for the whole organ. In cardiac exploration, the realization of ventricular and myocardic scintigraphies with systolic and diastolic selection allows a better detection of a ventricular ectasia or myocardic perfusion defects and dyskinetic motion. The determination of the ejection fraction can be made from the images by the area method. Pilotix has the advantage of simplicity for selecting isovolumetric phases with respect of cardiac rythm

Abstract (French)

L'examen scintigraphique constitue une accumulation d'informations affectee par le flou cinetique provoque par les organes en mouvements. On decrit la mise au point d'un module electronique eliminant les artefacts dus aux mouvements respiratoires et cardiaques. Le module electronique propose, 'Pilotix', est constitue de sondes recueillant les signaux electrocardiographique et de compliance respiratoires, d'un ensemble electronique selectionnant les phases de pause respiratoire et de fins de systole et de diastole de durees proportionnelles au rythme cardiaque. 'Pilotix' autorise la prise d'informations par les gamma-cameras pendant ces durees physiologiques choisies et prereglees par l'experimentateur. Les resultats cliniques montrent principalement en exploration abdominale une amelioration du pouvoir de resolution au niveau des contours des organes et des lacunes bordantes. Le mouvement respiratoire induit des translation, rotation et deformation de facons differentes dans un meme organe, rendant illusoires les corrections barycentriques. En exploration cardiaque, la realisation de scintigraphies en phases isovolumetriques permet une meilleure determination des mouvements et ectasies ventriculaires, des lacunes myocardiques et de la fraction d'ejection. 'Pilotix' peut servir d'interface a un ordinateur autorisant l'obtention des parametres de compliance cardiaque
