Highlights: • Characteristics of high-risk lesions on DCE, DWI and T2WI images have some significance in predicting upgrade risks. • In terms of malignant upgrade at subsequent surgery, DCE-MRI has a high NPV for women diagnosed with ADH. • Surgical resection is recommended for ADH and LN with a high upgrade rate and frequency on MRI. This article reviews the frequency, upgrade rate and valuable imaging characteristics for predicting the histologic upgrade risks of high-risk lesions on MRI, so as to provide a reference for the management of the lesions.
