Hemochromatotic liver and magnetic resonance imaging (MRI)

Description

A 63-year-old man, with no personal or hereditary family history of medical significance, consulted for liver cirrhosis. He had a serum ferritin > 1500 ng/ml, iron level 76 µg/dl, transferrin level 198 µg/dl, transferrin saturation index 38%. Antibodies for hepatitis A, B, and C viruses and human immunodeficiency virus were negative. On magnetic resonance imaging (MRI), the liver, with irregular borders, was decreased in size; the caudate lobe was normal. In spin-echo and T2-weighted sequences to assess iron overload, a diffuse decrease in signal ("black liver") was found, suggestive of high-grade iron deposits. The spleen was normal; there were multiple images of a serpiginous path compatible with splenic varices (arrowhead) and free fluid in the abdominal cavity. Genomic analysis revealed a heterozygous trait for the H63D gene. These findings were interpreted as consistent with hereditary hemochromatosis.

Hombre de 63 años, sin antecedentes personales ni heredofamiliares de trascendencia médica, consultó por cirrosis hepática. Tenía una ferritina sérica > 1 500 ng/ml, ferremia 76 µg/dl, transferrinemia 198 µg/dl, índice de saturación de transferrina 38%. Los anticuerpos para los virus de hepatitis A, B y C, y de la inmunodeficiencia humana fueron negativos. En las imágenes de resonancia magnética nuclear (RMN), el hígado, de bordes irregulares, tenía el tamaño disminuido; el lóbulo caudado era normal. En secuencias de spin eco y T2 pesado para valorar la sobrecarga de hierro, se encontró una disminución difusa de la señal ("hígado negro") sugestiva de depósitos de hierro de alto grado. El bazo era normal; había múltiples imágenes de trayecto serpiginoso compatibles con várices esplénicas (cabeza de flecha) y líquido libre en la cavidad abdominal. El análisis genómico manifestó un rasgo heterocigoto para el gen H63D. Se interpretaron estos hallazgos como compatibles con hemocromatosis hereditaria.
