Input of pre-operative radiomics to select patients with locally advanced rectal cancers

With the advent of total neoadjuvant therapy (TNT) for locally advanced rectal adenocarcinomas (LARC), high rates of pathologic complete response (pCR) are achieved, paving the way for organ preservation strategies. Identifying complete responders is a challenge. In this ancillary study of the GRECCAR 12 prospective multicentric randomized trial, we aimed to assess the input of baseline T2-weighted MRI radiomics associated with standard clinical and radiological data to predict pCR after neoadjuvant therapy. We retrospectively considered every patient included in the GRECCAR 12 trial. After pre-sampling of T2-weighted pretherapeutic MRI sequences, and a posteriori ComBat harmonization, 4 sets of 100 radiomic features were extracted from manually delineated tumors. Following random division between training and testing sets on a 3:1 ratio, univariate selection with stepwise regression and redundancy reduction, two models (with and without ComBat) using radiomic and non-radiomic variables were built. The prediction of pCR was assessed using logistic regression models, with ROC-AUC and accuracy of prediction as performance gauges. One hundred and sixty-one patients were included in the final radiomic analysis, 82 in the chemo-radiotherapy arm and 79 in the TNT arm. The rate of pCR was 46%. The best performing models including 2 non-radiomic features, ymrTRG and radiomic features (3 for No-ComBat and 4 for ComBat) obtained AUCs of 0.65 for No-ComBat and 0.74 for ComBat on the testing set, beating clinical variables (AUC of 0.56) or ymrTRG (AUC of 0.58) alone. Radiomics has the potential to better select patients for organ preservation, increasing pCR prediction accuracy in LARC patients after neoadjuvant therapy in association with clinical data and ymrTRG. Prospective trials and intergroup workforce are warranted, as well as evaluating the addition of other biomarkers (MSI status, circulating DNA...). (author)

INTRODUCTION: Grace a l'avenement du traitement neoadjuvant total (TNT) pour la prise en charge des adenocarcinomes du rectum localement avances, des taux eleves de reponse histologique complete (pCR) sont obtenus, ouvrant la voie a des strategies de preservation rectale. L'identification de repondeurs complets ou quasicomplets, uniquement a l'aide d'une evaluation clinique et radiologique, demeure cependant un defi. Bien que le ymrTRG soit couramment utilise comme substitut pour identifier les candidats eligibles, son pouvoir predictif est imparfait. Dans cette etude ancillaire de l'essai randomise multicentrique prospectif GRECCAR 12, nous avons cherche a evaluer l'apport de la radiomique extraite sur les IRM initiales en sequence T2, associee aux donnees cliniques et radiologiques classiques, pour predire la pCR apres traitement neoadjuvant. MeTHODES: Nous avons retrospectivement considere chaque patient inclus dans l'essai GRECCAR 12. Apres pre-echantillonnage (N4BiasReduction et Z-normalisation des intensites) des imageries IRM pre-therapeutiques ponderees en T2, et harmonisation ComBat a posteriori, 4 ensembles de 100 caracteristiques radiomiques (indices de forme et d'intensite de premier ordre, et parametres de texture de second ordre) ont ete extraits a partir des tumeurs delimitees manuellement pour chaque patient. Suite a une division aleatoire entre des ensembles d'entrainement et de test sur un ratio de 3:1, une selection univariee avec regression pas a pas et elimination des variables redondantes ont ete appliquees, menant a la creation de deux modeles (avec et sans ComBat) de regression logistique visant a predire la pCR. L'aire sous la courbe (AUC) ROC et le pourcentage d'exactitude de prediction ont ete utilises comme criteres de performance. RESULTATS: Cent soixante-six patients ont ete inclus dans l'analyse radiomique finale, 82 dans le bras chimiradiotherapie et 79 dans le bras TNT. Le taux de pCR dans la cohorte etait de 46%. La classification ymrTRG obtenait une AUC-ROC de 0.60, une exactitude de 61,5%, avec une sensibilite de 36,5% et une specificite de 82,5% pour predire la pCR dans la population globale. Nos meilleurs modeles de regression logistique, incluant 3 variables non-radiomiques (ymrTRG, taux d'antigene carcino-embryonnaire et bras de traitement) et des parametres radiomiques (3 pour le modele sans ComBat, 4 avec) obtenaient une AUC-ROC de 0.65 sans ComBat et 0.74 avec ComBat dans la cohorte de test, ameliorant les performances obtenues par le ymrTRG seul ou des modeles exclusivement cliniques. CONCLUSION: La radiomique a le potentiel d'ameliorer la selection des patients dans une optique de strategie de conservation rectale, ameliorant la prediction de la pCR chez les patients atteints de cancers rectaux localement avances en association avec les donnees cliniques et le ymrTRG. Des etudes prospectives ainsi qu'une collaboration intergroupe sont necessaires, ainsi que l'evaluation de l'apport d'autres biomarqueurs (statut MSI, ADN circulant,...). (auteur)
