Subepithelial lesions of the upper gastrointestinal tract: proposal for a management algorithm

Description

A review of current available literature on the concepts, diagnoses and approaches of subepithelial lesions (SELs) of the upper gastrointestinal tract was carried out in order to propose an algorithm for the diagnosis and management of SELs for Costa Rican hospitals. A detailed search for bibliographic information was carried out regarding the epidemiology, diagnosis, and management of subepithelial lesions in the upper gastrointestinal tract. A flow chart to manage subepithelial lesions of the upper gastrointestinal tract was proposed in order to make sequential and staggered management practical and simple for daily application. The flow chart was developed as an orientation for decision making, however, each case must always be individualized. Most SELs have been documented incidentally by endoscopy performed routinely for other symptoms. Leiomyomas have been found to be the most frequent SELs at the esophageal level and are generally benign. Digestive endoscopy has been the first step in the evaluation of SELs and has made it possible to describe the appearance, location and size of the lesion. Most of the malignant lesions have been hypoechoic and located in the third and fourth echolayers, making it necessary to perform a biopsy. The standard treatment for malignant SELs has been surgery, however new endoscopic management techniques have sought to reduce morbidity and mortality when resecting SELs

Abstract (Spanish)

Una revision de literatura actual disponible sobre los conceptos, diagnosticos y abordajes de las lesiones subepiteliales (LSE) del tracto gastrointestinal superior fue realizada con el fin de proponer un algoritmo de diagnostico y manejo de las LSE para los hospitales costarricenses. La busqueda detallada de informacion bibliografica fue realizada con respecto a la epidemiologia, el diagnostico y el manejo de las lesiones subepiteliales en el tracto gastrointestinal superior. Un flujograma para manejar lesiones subepiteliales del tracto gastrointestinal superior fue propuesto con el objetivo de que el manejo secuencial y escalonado sea practico y sencillo para la aplicacion diaria. El flujograma fue desarrollado como orientacion para la toma de decisiones, sin embargo, cada caso siempre se debe individualizar. La mayoria de LSE han sido documentadas de forma incidental mediante endoscopia efectuada de forma rutinaria por otros sintomas. Los leiomiomas han sido encontrados como las LSE mas frecuentes a nivel esofagico y generalmente son benignas. La endoscopia digestiva ha sido el primer paso en la evaluacion de las LSE y ha permitido describir aparencia, localizacion y tamano de la lesion. La mayoria de lesiones malignas han sido hipoecoicas y localizadas en la tercera y cuarta ecocapas, siendo necesario realizar una toma de biopsia. El tratamiento estandar para las LSE malignas ha sido la cirugia, sin embargo nuevas tecnicas de manejo endoscopico han buscado disminuir la morbilidad y mortalidad al resecar LSE
