Radiodermatitis. Etiological and medico-legal considerations

This study covers 22 radiodermatitis cases recorded in and around Paris and in industry, from January 1st 1967 to mid-1977, by the health insurance treasury of te Paris region. Special attention was paid to the analysis of their frequency, seriousness and causes and of the medico-legal problems involved. According to our observations these accidents mainly affect radiometallographers, crystallographers, people studying materials by X-fluorescence and technicians responsible for maintenance of medical X-ray radiotherapy machines, the activities most prevalent in the industrial use of ionizing radiations. Three main accident factors are observed: imperfections in the protection system and in the maintenance of equipment; carelessness of personnel towards ionizing radiations owing to the apparent absence of immediate effects; lack of information on the danger of ionizing radiations and on the defensive measures necessary. Three means of protection must be developed as a result: wearing of an extra dosimeter on the wrist or finger; permanent information of staff; checks on equipment and constant improvement in the protection of particularly dangerous instruments (gammagraphs). Acute and chronic radiodermatitis are taken in charge as occupational diseases since they appear in table six of these illnesses with a coverage period of 60 days and 10 years. Radiodermatitis patients face the question of professional reclassification, a difficulty all the greater as their qualifications are high. The possibility of paid professional training for occupational disease victims is especially useful when the subjects are still young

Notre etude porte sur 22 radiodermites survenues dans la region parisienne et dans l'industrie du premier janvier 1967 a la fin du premier semestre 1977; celles-ci ont ete recensees par la caisse d'assurance maladie de la region parisienne. Nous avons analyse plus particulierement sur leur frequence, leur gravite, leurs etiologies et les problemes medico-legaux qu'elles soulevent. Ces accidents touchent dans notre observation essentiellement des radiometallographes, des cristallographes, des sujets etudiant des materiaux par fluorescence X et des technieins charges de l'entretien d'appareils de radiologie medicale et de radiotherapie; ces postes sont les plus frequemment rencontres parmi les utilisations industrielles des rayonnements ionisants. Trois facteurs d'accidents sont surtout observes: - imperfections dans le systeme de protection et dans l'entretien du material, - imprudence de l'homme vis-a-vis des rayonnements ionisants dont il ne ressent pas immediatement les effets, - manque d'information sur le danger des rayonnements ionisants et sur les moyens de s'en premunir. Trois moyens de protection a developper en decoulent; - port d'un dosimetre complementaire au poignet ou au doigt; - information permanente des salaries; - controle du material et amelioration constante de la protection des appareils particulierement dangereux (gammagraphes). En dehors des cas accidentels, les radiodermites aigues et chroniques sont prises en charge au titre des maladies professionnelles puisqu'elles figurent au sixieme tableau de ces maladies avec un delai de prise en charge de 60 jours et 10 ans. Chez les sujets atteints de radiodermites se pose le probleme du reclassement professionnel qui est d'autant plus delicat qu'ils sont plus qualifies. La possibilite d'une formation professionnelle remuneree chez les sujets pris en charge au titre des maladies professionnelles est particulierement utile chez les victimes encore jeunesFiles
