Iridium 192 endocurietherapy for epidermoid carcinoma of tonsillar region

From July 1971 to October 1984, 42 patients with T1 or T2 tumors of the tonsillar region were treated according to the following protocol: 1. Telecobaltherapy to the primary site and the neck nodes to a dose of 45 Gy. 2. Brachytherapy to the primary site to a dose of 25 to 30 Gy using iridium 192. 3. For node positive patients, boost dose to involved neck nodes with electrons, or neck dissection. Three patients with T1NO tumors were treated exclusively by iridium implantation. The crude disease free survival is 66% at 5 years and 50% at 10 years. Local control is 98%. Regional control is 100% for the NO group and 94% for the N1-3 group. Four cases of soft tissue ulceration were observed, all of which healed spontaneously within a few months. Salivary impairment was minimal. While these promising results have encouraged the use of this protocol, the introduction of the plastic tube technique has expanded the indications to include almost all T1 and T2 tumors of the tonsillar region without obvious extension to the base of tongue or retromolar trigone
Abstract (French)
De juillet 1971 a octobre 1984, 42 T1 et T2 de la region amygdalienne ont ete traites par le protocole suivant: 1. Teleradiotherapie de la tumeur primitive et des aires ganglionnaires de 45 Gy. 2. Endocurietherapie de la tumeur primitive par iridium 192 de 25 a 30 Gy. 3. Complement d'irradiation des sites ganglionnaires initialement envahis par electrons a une dose comprise entre 25 et 30 Gy ou evidemment ganglionnaire cervical. Trois tumeurs classees T1NO ont ete traitees exclusivement par endocurietherapie. La survie sans signe evolutif est de 66% a 5 ans et de 50% a 10 ans. Le controle local est de 98%. Le controle des aires ganglionnaires est de 100 % pour la population des malades NO et de 94% pour celle des malades N1N2N3. Les complications se sont limitees a 4 ulcerations muqueuses qui ont cicatrise en quelques mois. Les sequelles salivaires sont reduites par comparaison a celles d'une teleradiotherapie exclusive. L'introduction recente d'une nouvelle technique d'endocurietherapie permet de recommander la methode pour presque tous les T1 et T2 de la region amygdalienne sans infiltration franche de la base de langue ou de la commissure intermaxillaireAdditional details
