The role of pharmacists in nuclear risk management: a survey of pharmacies in high-risk areas

Nuclear power is France's main source of electricity. In the event of a major accident, people living around nuclear power plants would be the first to be affected. For this reason, the prefects' action plan is twofold: to take iodine tablets to saturate the thyroid gland, and to confine or evacuate residents. The dispensing pharmacist secures the dispensing of the tablets and relays information on what to do in the event of a nuclear accident. We conducted a qualitative survey in the form of semi-structured interviews with pharmacies located within a 10 to 20 kilometer radius. The aim was to gather feedback from professionals on the conduct of the distribution campaign organized in 2019. Our survey covered three main areas: training and/or information received by pharmacy teams; organization of the campaign; and areas for improvement. Several findings emerged. Firstly, access to information varied both between regions and within individual pharmacies. Secondly, the management and organization of the campaign needs to be improved, particularly in terms of enhancing the role of pharmacy teams and ensuring traceability of dispensations. We have also identified that general advice on what to do in the event of a nuclear accident is rarely given. More substantial information and training measures are needed to reinforce the involvement of pharmacies in campaigns and ensure optimum patient protection. It would also be useful to get feedback from patients on how they see the pharmacist's role in campaigns, and what their expectations are
Abstract (French)
L'energie nucleaire est la principale source d'electricite en France. En cas de defaillance majeure, les populations residant autour des centrales seraient les premieres impactees. Pour cela, le plan d'action des prefets se decline en deux axes: la prise de comprimes d'iode permettant la saturation de la thyroide et le confinement voire l'evacuation des habitants. Le pharmacien d'officine securise la dispensation des comprimes et est le relais d'informations sur la conduite a tenir en cas d'incident nucleaire. Nous avons realise une enquete qualitative sous forme d'entretiens semi-directifs aupres de pharmacies situees dans le perimetre de 10 a 20 kilometres. L'objectif etait de recueillir les retours d'experience des professionnels sur le deroulement de la campagne de distribution organisee en 2019. Notre enquete comportait trois grands themes: la formation et ou les informations recues par les equipes officinales; l'organisation de la campagne; les axes d'ameliorations identifies. Plusieurs constats ont emerge. Tout d'abord, il existe une variabilite dans l'acces a l'information, tant au niveau des regions qu'au sein des officines. Deuxiemement, la gestion et l'organisation de la campagne necessitent une amelioration notamment en ce qui concerne la valorisation du role des equipes officinales et la tracabilite des dispensations. Nous avons egalement identifie que les conseils generaux sur la conduite a tenir en cas d'incident nucleaire sont peu abordes. Des mesures plus substantielles d'information et de formation sont necessaires pour renforcer l'implication des officines lors des campagnes et assurer une protection optimale des patients. Par ailleurs, il serait pertinent d'avoir le retour des patients quant a leur vision de la place du pharmacien dans les campagnes et leurs attentes. (auteur)
