Events in radiation protection - To analyse errors and to improve practices. An action plan is set after an incident. The error, a symptom of a system dysfunction

Description

While providing data regarding significant events in radiation protection (numbers between 2010 and 2017, distribution among category and among activity in 2017), a first article evokes the definition of these events and describes how these events are processed by the ASN and the IRSN French organizations. This process comprises an assessment of dose received by organs, and a statement of lessons learned. As an example, the article describes how a barometer failure was at the origin of an incident. The second article describes how an action plan has been set after an event which occurred in a hospital. The last article outlines that significant radiation protection events can be a consequence of a defaulting organisation. Thus, it outlines that the analysis of these events from the perspective of organisational and human factors is necessary
