Long term follow-up results of high-dose rate remote afterloading intracavitary radiation therapy for carcinoma of the uterine cervix

Between August 1978 and December 1982, a total of 200 patients with carcinoma of the intact uterine cervix were treated using HDR. According to the staging system of UICC (1987), 8 patients were classified into Stage Ia, 22 Ib, 22 IIa, 53 IIb, 85 III, and 10 IV. Cause specific five- and ten-year survival rates by Stage were 100%, 100% in Stage Ia, 90%, 90% in Stage Ib, 76% in Stage IIa, 86%, 84% in Stage IIb, 54%, 46% in Stage III and 20%, 20% in Stage IV, respectively. Significant prognostic factors by Cox's multivariate analysis were stage, the value of hemoglobin and older age. Concerning stage classification the value of hemoglobin was the most important prognostic factor in Stage II and Stage III. Late complications requiring medical treatment after RALS developed in 24 patients (12%), i.e., rectum 14 (7), bladder 8 (4), small intestine 5 (3), sigmoid colon 1 (1). Severe complications requiring surgery were noted in 9 patients (5%). HDR yields good results of survival with a low risk of sivere side effects for the treatment of carcinoma of uterine cervix based on our long-term follow-up results. (orig./MG)

Zwischen August 1978 und Dezember 1982 wurden insgesamt 200 Patienten mit Zervixkarzinomen primaer mittels HDR behandelt. Entsprechend der Stadieneinteilung der UICC (1987) wurden acht Patienten dem Stadium Ia, 22 dem Stadium Ib, 22 dem Stadium IIa, 53 dem Stadium IIb, 85 dem Stadium III und zehn dem Stadium IV zugeordnet. Die tumorspezifischen Fuenf- und Zehn-Jahres-Ueberlebensraten entsprechend des Stadiums waren 100% bzw. 100% im Stadium Ia, 90% bzw. 90% im Stadium Ib, 76% bzw. 76% im Stadium IIa, 86% bzw. 84% im Stadium IIb, 54% bzw. 46% im Stadium II und 20% bzw. 20% im Stadium IV. Signifikante Prognosefaktoren entsprechend dem Cox-Modell waren Stadium, Haemoglobinwert und Alter. Bezueglich der unterschiedlichen Stadien war der Haemoglobinwert der wichtigste prognostische Faktor fuer das Stadium II und Stadium III. Behandlungsbeduerftige Spaetkomplikationen nach Afterloading-Therapie entwickelten sich bei 24 Patienten (12%), und zwar in Rektum 14 (7), Blase 8 (4), Duenndarm 5 (3) und Sigma 1 (1). Schwere Komplikationen, die eine Operation erforderten, wurden bei neun Patienten (5%) beobachtet. Auf der Grundlage der Langzeitergebnisse ergibt HDR-Afterloading gute Ergebnisse fuer das Ueberleben mit einem niedrigen Risiko schwerer Nebenreaktionen bei der Behandlung des Zervixkarzinoms.
