Contribution of [18F]FET PET in the management of gliomas, from diagnosis to follow-up: a review

Gliomas, the most common type of primary malignant brain tumors in adults, pose significant challenges in diagnosis and management due to their diverse and aggressive nature. This review evaluates the utility of O-(2-[18F]fluoroethyl)-L-tyrosine ([18F]FET) positron emission tomography (PET), a promising imaging modality, to enhance the clinical management of gliomas.We reviewed 82 studies involving 4657 patients, focusing on the application of [18F]FET in several key areas: diagnosis, grading, identification of IDH status and presence of oligodendroglial component, guided resection or biopsy, detection of residual tumor, guided radiotherapy, detection of malignant transformation in low-grade glioma, differentiation of recurrence versus treatment-related changes and prognostic factors and treatment response evaluation. Our findings confirm that [18F]FET helps delineating tumor tissue, improves diagnostic accuracy and aids in therapeutic decision-making by providing crucial insights into tumor metabolism. This review underscores the need for standardized parameters and further multicentric studies to solidify the role of [18F]FET PET in routine clinical practice. By offering a comprehensive overview of current research and practical implications, this paper highlights the added value of [18F]FET PET in improving management of glioma patients from diagnosis to follow-up. Abstract (French)

Les gliomes, le type le plus courant de tumeurs cerebrales malignes primitives chez l'adulte, posent des defis importants en matiere de diagnostic et de prise en charge en raison de leur nature diversifiee et agressive. Cette revue evalue la contribution de la tomographie par emission de positons (TEP) a l'O-(2- ["1"8F]fluoroethyl)-L-tyrosine (["1"8F]FET), une modalite d'imagerie prometteuse, pour ameliorer la prise en charge clinique des gliomes. Nous avons examine 82 etudes impliquant 4657 patients, se concentrant sur l'application du ["1"8F]FET dans plusieurs domaines cles: diagnostic, gradation, determination du statut IDH et presence d'un composant oligodendroglial, resection ou biopsie guidee, detection de tissu tumoral residuel, radiotherapie guidee, detection d'une transformation maligne dans les gliomes de bas grade, differenciation de la recidive versus changements post-therapeutiques, facteurs pronostiques et evaluation de la reponse au traitement. Nos resultats confirment que la ["1"8F]FET aide a delimiter le tissu tumoral, ameliore la precision du diagnostic et facilite la prise de decision therapeutique en fournissant des informations cruciales sur le metabolisme tumoral. Cette revue souligne la necessite de parametres standardises et le besoin d'etudes multicentriques pour consolider le role de la TEP ["1"8F]FET dans la pratique clinique de routine. En offrant un apercu complet des recherches actuelles et de leurs implications pratiques, cet article met en evidence la valeur ajoutee du ["1"8F]FET PET dans l'amelioration de la prise en charge des patients atteints de gliome, du diagnostic au suivi.
