A prospective randomized study concerning the point a dose in high-dose rate intracavitary therapy for carcinoma of the uterine cervix. The final results

Between January 1983 and February 1989, a total of 165 patients with carcinoma of the unterine cervix was entered in a prospective randomized study concerning the point A dose of HDR therapy (6 Gy/fraktion vs 7.5 Gy/fraction) and external irradiation dose at Department of Radiation Therapy, The Center for Adult Diseases, Osaka. UICC stage distribution of patients was as follows: Stage IA=4, stage IB=33, stage IIA=18, stage IIB=38, stage III=57, stage IV=15. Overall 5-year cause specific survivals were as follows: Stage IA=100%, stage IB=96%, stage IIA=92%, stage IIB=79%, stage III=57%, stage IV=27%. In each stage, 5-year survival rates in groups A and B were 100%, 93% in stage I, 82% and 85% in stage II, 62% and 52% in stage II and 22% and 31% in stage IV, respectively. There were no statistically significant differences among these survival curves in each stage. Five-year local failure rates were 16% in group A and 16% in group B (p=0.9096), and corresponding distant failure rates were 23% in group A and 19% in group B (p=0.2955). Moderate-to-severe complications requiring treatment (Kottmeier's grade 2 or more) were noted in 6 patients (7%) in group A and 6 patients (7%) in group B. All of the bladder and rectal complications needed medical treatment (Kottmeier's grade 2). Severe complications receiving surgery were noted in 4 patients (A: 1; B: 3), i.e., small intestine 3 and sigmoid colon 1 patient. Another 1 patient (A) was dead of ileus. There were no statistically significant differences between 2 treatment schedules in survival rates, failure patterns and complications rates. This fact suggests that small number of fractions (7.5 Gy/fraction) may be advantageous because of short duration and a low load of treatment. (orig.)
Abstract (German)
Von Januar 1983 bis Februar 1989 wurden 165 Patientinnen mit Zervixkarzinomen in der Strahlentherapeutischen Klinik in Osaka in einer prospektiven randomisierten Studie behandelt. Die Patienten erhielten unterschiedliche Fraktionsdosen der HDR-Therapie am Punkt A, stadienabhaengig 3- bis 6mal 6 Gy oder 3- bis 5mal 7,5 Gy bei jeweils einer Fraktion pro Woche. Alle Patienten wurden auch perkutan bestrahlt mit Dosen von 40 bis 60 Gy in 2-Gy-Einzeldosis, fuenf Fraktionen pro Woche, davon 10 bis 40 Gy offen ohne Mittelblock. Entsprechend der Stadieneinteilung nach UICC lag in vier Faellen ein Stadium IA, 33mal ein Stadium IB, 18mal ein Stadium IIA, 38mal ein Stadium IIB, 57mal ein Stadium III und 15 mal ein Stadium IV vor. Die krankheitsspezifischen Fuenf-Jahres-Ueberlebensraten betrugen: Stadium IA 100%, Stadium IB 96%, Stadium IIA 92%, Stadium IIB 79%, Stadium III 57%, Stadium IV 27%. Die Fraktionsdosis am Punkt A (6 Gy vs. 7,5 Gy) hatte keinen Einfluss auf die Fuenf-Jahres-Ueberlebensraten (100% vs. 93% im Stadium I, 82% vs. 85% im Stadium II, 63% vs. 52% im Stadium III und 22% vs. 31% im Stadium IV). Auch die Lokalrezidivraten nach fuenf Jahren (16% vs. 16%) sowie die Rate von Fernmetastasen (23% vs. 19%) waren bei beiden Fraktionierungsregimen gleich. Die Rate schwerer Nebenwirkungen (Grad II oder mehr nach Kottmeier) war in beiden Fraktionierungsregimen ebenfalls gleich, sie betrug 7%. Insgesamt vier Patienten zeigten operationspflichtige Darmkomplikationen, ein Patient verstarb an einem Ileus. In dieser randomisierten Studie zeigte sich kein Unterschied hinsichtlich Gesamtueberleben, lokaler Kontrolle und Komplikationsrate bei HDR-Fraktionsdosen von 6 bzw. 7,5 Gy. Die hoehere Fraktionsdosis koennte deshalb bei gleichen Behandlungsergebnissen aufgrund der insgesamt geringeren Fraktionszahl vorteilhafter sein. (orig.)Additional details
