Expert systems in clinical practice
Description
The first expert systems prototypes intended for advising physicians on diagnosis or therapy selection have been designed more than ten years ago. However, a few of them are already in use in clinical practice after years of research and development efforts. The capabilities of these systems to reason symbolically and to mimic the hypothetico-deductive processes used by physicians distinguishes them from conventional computer programs. Their power comes from their knowledge-base which embeds a large quantity of high-level, specialized knowledge captured from medical experts. Common methods for knowledge representation include production rules and frames. These methods also provide a mean for organizing and structuring the knowledge according to hierarchical or causal links. The best expert-systems perform at the level of the experts. They are easy to learn and use, and can communicate with the user in pseudo-natural language. Moreover they are able to explain their line of reasoning. These capabilities make them potentially useful, usable and acceptable by physicians. However if the problems related to difficulties and costs in building expert-systems are on the way to be solved within the next few years, forensic and ethical issues should have to be addressed before one can envisage their routine use in clinical practice

Il y a plus de quinze ans qu'ont ete developpes les premiers systemes experts appliques a l'aide a la decision medicale et certains d'entre eux commencent a etre utilises en pratique clinique. Ils constituent une approche nouvelle de l'aide a la decision medicale assistee par ordinateur, et utilisent des capacites de raisonnement symbolique proche du mode de raisonnement hypothetico-deductif des medecins, qui les differencient des programmes informatiques traditionnels. Leur puissance est liee a la qualite de leur base de connaissances, qui contient une grande quantite de connaissances de haut-niveau, tres specialisees dans un domaine etroit et obtenues a partir de medecins experts. Ces connaissances sont representees habituellements sous forme de regles de production et de cadres, qui permettent a l'expert d'introduire ses connaissances dans le systeme en langage quasi-naturel, et de les structurer selon des liaisons hierarchiques ou causales. Les meilleurs de ces systemes ont des performances equivalentes a celles des experts qui les ont concus. Leur facilite d'utilisation, liee a leur capacite a communiquer avec l'utilisateur en langage naturel et leur possibilite d'expliciter leur raisonnement les designent comme des outils potentiellement utiles, acceptables et utilisables par les medecins. Si les problemes lies aux difficultes et au temps de developpement de ces systemes doivent pouvoir etre resolus dans les prochaines annees, il reste a aborder les problemes ethiques et medico-legaux que souleverait leur utilisation en pratique cliniqueAdditional details
