[en] Gallium-67 scanning has been widely used in the diagnosis of malignant diseases or inflammatory foci. Only few attempts have been made to evaluate this investigation in sarcoidosis. A whole body scan was performed according to a same schedule for 27 patients with sarcoidosis and for 36 controls investigated for abdominal abscess. Mediastinal, pulmonary and salivary uptake were noted only in sarcoidosis. Uptake in the region of the eyes was seen in controls but appeared more frequently and with higher intensity in sarcoidosis. Nose and spleen uptake was more difficult to deal with. Although suffering from a lack of specificity, gallium scanning improves the localisation of granulomatous lesions and shows as yet unknown regions involved with sarcoidosis

Le gallium-67 a beaucoup ete utilise pour le diagnostic d'affections carcinologiques ou d'abces profonds. Par contre, peu de travaux ont tente de degager l'interet de cet examen dans la sarcoidose. Une scintigraphie corps entier a ete effectuee selon le meme protocole chez 27 malades atteints de sarcoidoses et 36 sujets temoins examines pour suspicion d'abces abdominal. Des images de fixation mediastinales, pulmonaires et salivaires n'ont ete observees que dans les sarcoidoses. Des images de fixation oculaires existent chez les temoins, mais elles sont plus frequentes et surtout plus intenses au cours de la sarcoidose. Enfin, les images de fixation nasales et spleniques sont d'interpretation plus difficile. Bien que depourvue de specificite, la scintigraphie au gallium permet un reperage facile des lesions granulomateuses; elle peut mettre en evidence des localisations jusque-la meconnues. AbstractAbstract
[en] Substructures similar to those previously reported [1] were observed near an isobaric analogue resonance in 67Ga. The results of an analysis regarding the significance of these substructures against the statistical reaction model are presented. It follows that with a high degree of confidence these substructures are not of statistical origin. (author)

AbstractAbstract
[en] 67-Ga imaging was performed in 8 treated and 6 untreated patients with documented South American Blastomycosis (paracoccidioidomycosis) confirmed by histology. All patients were male, aged 20-68 years, with evolution of the disease from two months to 18 years. Sorologic tests were performed in 12 patients. 67-Ga images were correlated with X-ray in all patients. The images with 67-Ga showed evidency of lesions unsuspected by clinical examination and missed by X-ray in 9 patients. On the other hand, two cases with clinically diagnosed tegumentar lesions, particularly of the oro-nasal region, were not demonstrated on 67-Ga images. Follow-up studies after specific therapy were done in four patients and demonstrated concordance between X-ray and 67-Ga in two patients with pulmonary involvent; in the other two patients with lymphatic system involvement these studies showed normalization of the 67-Ga images although their sorologic tests persisted positive. This initial evaluation demonstrated that 67-Ga imaging is a useful tool for detection of suspected and, more importantly, unsuspected lesions of paracoccidioidomycosis and for evaluation of the response of the disease to specific therapy. O estudo de imagens com 67-Ga foi realizado em 14 pacientes (oito tratados e seis virgens de tratamento) com blastomicose sul-americana (paracoccidioidomicose) confirmada pela histologia. Todos os pacientes eram masculinos, com idades entre 20 e 68 anos, com evolucao de doenca entre dois meses e 18 anos. A avaliacao sorologica foi efetuada em 12 pacientes. As imagens com 67-Ga foram correlacionadas com o estudo radiologico em todos eles e mostraram evidencias de lesoes nao suspeitadas pelo exame clinico ou nao mostradas pela radiologia em nove casos: parotidas em quatro casos; testiculos em dois; ossos em um; ganglio mediastinal em um e ganglio cervical mediano profundo (que causou obstrucao do ducto toracico) em outro. Em contrapartida, dois casos com lesoes clinicamente evidentes no tegumento (particularmente na regiao oro-nasal) nao foram mostrados no estudo com 67-Ga. No acompanhamento apos tratamento especifico em quatro pacientes, houve concordancia entre os estudos radiologicos e com 67-Ga em dois, com envolvimento pulmonar; em outros dois, com envolvimento ganglionar, houve normalizacao nas imagens com 67-Ga enquanto a sorologia continuava positiva. Esta avaliacao inicial demonstrou que as imagens com 67-Ga sao uteis na deteccao de lesoes suspeitas e, muito mais importante, naquelas insuspeitas da blastomicose, bem como para avaliar a resposta de doenca ao tratamento especifico. [en] Sarcoidosis is a multisystemic disease and has diverse presentations which makes the diagnosis a little difficult. Gallium is being used to diagnose active sites of involvement. Whole body can be imaged by gallium scintigraphy using a gamma camera aligned to a moving table and tomographic camera. [en] A method is described for 67Ga-labeling liposomes containing a polyethylene glycol coating which exhibit prolonged blood circulation, reduced liver and spleen uptake and accumulation in tumours. Applications as agents for diagnostic imaging and delivery of therapeutic agents are considered. Previous methods were adapted to compensate for the presence of low temperature phase transition phospholipids resulting in consistent loading with low levels of residual unentrapped label. (author)

AbstractAbstract
[en] Sarcoidosis is a chronic multisystem disorder of unknown etiology characterized by non-caseating epitheloid granulomas. We report a case of a 29-year old female with acute sarcoidosis and Heerfordt's syndrome (parotid enlargement, anterior uveitis and facial nerve palsy) who had a typical clinical constellation but no histologic criteria of a granulomatous inflammatory process in transbronchial lung biopsy. 67Ga scintigraphy revealed such a typical pattern of accumulation that sarcoidosis could be diagnosed despite negative histological findings. Die Sarkoidose ist eine chronische Systemerkrankung unbekannter Aetiologie, die durch nichtverkaesende epitheloidzellige Granulome gekennzeichnet ist. Wir berichten ueber eine 29jaehrige Patientin mit akuter Sarkoidose und Heerfordt-Syndrom (Parotisschwellung, Iridozyklitis und Fazialisparese), bei der zwar eine typische klinische Befundkonstellation vorlag, die transbronchiale Lungenbiopsie jedoch histologisch keinen granulomatoesen Entzuendungsprozess nachweisen konnte. Die 67Ga-Szintigraphie zeigte ein so typisches Anreicherungsmuster, dass trotz negativer Histologie die Diagnose gestellt werden konnte. (orig.)Original Title
