[en] In idiopathic Parkinson's disease (PD) postganglionic involvement of the autonomic nervous system predominates, whereas in multiple system atrophy (MSA) preganglionic structures are mainly affected. Since cardiac I-123 metaiodobenzylguanidine (MIBG) scintigraphy can assess functional integrity of postganglionic sympathetic neurons, we evaluated its ability to differentiate PD and MSA. Cardiac MIBG scintigraphy was performed on 47 patients (26 PD and 21 MSA; 25 males, 62±9 yr) and 8 normal controls (NC: 3 males, 45±9 yr). All patients were excluded for myocardial ischemia by TI-201 SPECT. MSA patients were predominantly parkinsonism (MSA-p) and cerebellar subtypes (MSA-c) in 11 and 10 cases, respectively. Early (20 min) and delayed (4 hr) images were used to obtain heart/mediastinum count (H/M) ratios and washout rates (WRs). The both early (E) and delayed (D) H/M ratios were significantly decreased in PD (E; 1.54±0.23, D: 1.36±0.24) as well as MSA (E; 1.80±0.44, D; 1.88±0.51) patients compared to NC (E; 2.74±0.97, D; 2.94±1.20) (all p<0.01). The PD group had a significantly lower H/M ratio than the MSA group (E; p = 0.03, D; p = 0.003). The WRs of the PD patients (59.5±15.4%) were significantly increased compared to MSA patients and NC (26.6±23.9% and 19.7±11.5%, both p<0.0001). The WRs between MSA patients and NC were not significantly different. In the differential diagnosis of the PD and MSA, the sensitivity, specificity and accuracy by early H/M ratio (cut-off = 1.80) were 84.6% (22/26), 52.5% (11/21), and 70.2% (33/47), respectively, by delayed H/M ratio, 100.0% (26/26), 66.7% (14/21), and 85.1% (40/47), respectively, and by WR (cut-off 40.0%), 92.3% (24/26), 76.2% (16/21), and 85.1% (40/47), respectively. While the delayed H/M ratios were below 1.80 in 5/11 MSA-p patients, 1 of those 5 MSA-p patients had a decreased WR. [en] 16α iodo estradiol has been demonstrated to have as good an affinity as estradiol for estrogen-receptors and, labeled with iodine 123, may provide a good scanning agent fot visualisation of tissues containing estrogen-repectors, especially mammary tumors. 123I-16α iodo estradiol has been synthesized by an halogen exchange of 16ν bromo estradiol according to the procedure described by Hochberg for 125I-16α iodo estradiol labeling. Radiochemical yields are much lower than with iodine 125 (1 to 30%) and extremely variable. Specific activity range from 1,000 to 2,000 Ci/mmole

Le 16α iodo estradiol (IE2) presente une affinite pour les recepteurs d'estrogenes comparable a celle de l'estradiol et on peut envisager de l'utiliser, marque a l'iode-123 pour la visualisation de tissus riches en recepteurs d'estrogenes, notamment certaines tumeurs mammaires. On a synthetise le 123I-16α iodo estradiol par substitution du brome par l'iode sur le 16ν bromo estradiol suivant la technique decrite par Hochberg pour le marquage a l'iode-125. [en] No-carrier-added (nca) MIBG is primarily associated with specific uptake (i.e. uptake-1 mechanism). We evaluated the hypothesis that nca MIBG will be less influenced by changes in extra-neuronal uptake (i.e. uptake-2 mechanism) compared with carrier-added (ca) MIBG. No-carrier-added MIBG was compared with ca MIBG of two different manufacturers (ca MIBG-1 and ca MIBG-2, with a specific activity of 200 Mq/μmol and 40 MBq/μmol MIBG respectively) in rats (n=6 per group): controls, blocking uptake-1 (desipramine) and blocking uptake-2 (phenoxybenzamine hydrochloride). Dedicated pinhole SPECT was performed 2 h after injection of the radiotracer. After SPECT, biodistribution was assessed [% injected dose per gram tissue (%ID)]. No-carrier-added MIBG had the highest absolute cardiac uptake. Although a clear trend was observed, nca MIBG was not statistically significantly different from ca MIBG-1 (0.31±0.05 %ID vs 0.25±0.01 %ID,p=0.05). Blocking uptake-1 resulted in a significant decrease in absolute cardiac uptake only for nca MIBG (0.22±0.03 %ID,p=0.004). Blocking uptake-2 resulted in a significant reduction in ca MIBG-1 cardiac uptake (0.14±0.02 %ID,p=0.0001), but not in the cardiac uptake of nca MIBG or MIBG-2. SPECT showed the highest relative cardiac uptake for nca MIBG. Poor contrast between myocardium and surrounding tissue hampered assessment of relative cardiac uptake on SPECT of both ca MIBG-1 and ca MIBG-2. No-carrier-added MIBG yields a higher myocardial uptake than ca MIBG and is associated with a higher specific as well as a lower non-neuronal uptake. We therefore conclude that for the scintigraphic assessment of the myocardial sympathetic nervous system, nca MIBG is to be preferred over ca MIBG. (orig.)
