AbstractAbstract
[en] The results are presented of computerized tomography (CT) diagnosis of degenerative lumbar spine changes based on 6 years of experience with CT scanning. Diagnostic possibilities are especially defined for intervertebral disk prolapses. In compliance with the data in the literature, the significance is emphasized of CT which should become a prevalent radiodiagnostic method for the vertebrogenic lumboischiadic syndrome, especially in view of its high sensitivity. Wherever available, CT scanning should be a satisfactory indication for surgical or conservative therapy. However, CT scanning is not commonly available and thus, perimyelography (PMG) remains the method of choice. Nevertheless, CT scanning is necessary should surgery be considered, in all cases where PMG is disadvantageous or contraindicated, or where its results are not unambiguous. [en] Successful patient care involves interdisciplinary Cupertino. Images allow an interpretation of a static dimension, but may not be sufficient to develop a surgical treatment strategy, since many shoulder problems have its origin in a dynamic pathology. This article outlines dynamic clinical tests of shoulder pathology, classifies various degenerative and posttraumatic shoulder problems and stresses key facts an imaging report should include in order to plan surgery. This article conveys basic knowledge of those tests and the dynamic background of shoulder pathology. Basic surgical treatment principles are summarised briefly

AbstractAbstract
[en] Basal cell carcinoma (BCC) is the most common skin tumor and contains several different histopathological types. Here, we report a case of cystic basal cell carcinoma, which is relatively rare and might be clinically misdiagnosed. A dermatoscopic examination of the case revealed a homogenous blue/black area usually not seen in BCC. We reviewed 102 BCC cases resected and diagnosed at Sapporo Medical University Hospital between April 2005 and March 2010. Among them, only three were the cystic type.

[en] The purpose of this study was to evaluate the specialized Orthopantomograms and standard films compared with Water s' views in the diagnosis of maxillary sinusitis. The author analysed the types of mucosal thickening, the types of bony wall change, the relation between the types of chief complaints and mucosal thickening on the Waters' views of 156 patients with 206 maxillary sinuses demonstrating radiographic changes. And the pathologic change of antral floor was observed on 187 standard at 156 specialized Orthopantomogams and 187 standard films. The results were obtained as followings. 1. Among 206 mucosal thickening classified by 6 types, generalized increased radiopacity was 69 cases (33.5%) and the entire thickening of mucosa except an ostium area was 66 cases (32.0%). 2. The bony wall change was seen in 128 cases (62.1%) and the incidence of indistinct antral wall was the highest as 55 cases (43.0%) among 5 types. 3. Among 148 chief complaints, pain was 71 cases (48.0%), swelling was 19 cases (12.8%) and discomfort was 18 cases (12.2%). 4. The most frequent change of antral floor was indistinct antral floor as 25 cases (13.4%) and elevation of antral floor was 23 cases (12.3%). 5. In 156 specialized Orthopantomograms, 101 cases (49.0%) presented apparent increased radiopacity and 72 cases (35.0%) showed suspicious increased radiopacity in comparison with Waters' view. In 187 standard films, 47 cases (25.1%) presented apparent increased radiopacity and 53 cases (28.4%) showed suspicious increased radiopacity in comparison with Waters' view.

[en] Impaction of tooth is a situation in which an unerupted tooth is wedged against another tooth or teeth or otherwise located so that it cannot erupt normally. The supernumerary tooth is also called as hyperdontia and defined as the condition of having additional tooth to the regular number of teeth. The most common supernumerary tooth is a mesiodens, which is a mal-formed, peg-like tooth that occurs between the maxillary incisors. The supernumerary tooth is commonly impacted but they are frequently impacted on maxilla. Ectopic impaction of supernumerary tooth on mandibular condyle, coronoid process, ascending ramus, and pterygomandibular space is very rare condition. In this case, we report a case of impacted supernumerary tooth on mandibular sigmoid notch without definite pathologic change.

AbstractAbstract
[en] The classic radiographic presentation of multiple myeloma is lytic skeletal lesions. Primary sclerotic manifestations are rare and occur only in 3 % of cases. The imaging spectrum in three cases of multiple myeloma with primary osteosclerosis is described. The first patient had spiculated sclerosis of the orbit, which is an uncommon site for myeloma. The second patient with POEMS syndrome had multiple, scattered, skeletal lesions with sclerotic margins. The third patient presented with a chest wall mass and had an expansile thick spiculated sclerosis in the rib. The wide imaging spectrum possible in sclerotic myelomas and their relevant differential diagnosis is emphasized. (orig.)
