[en] This work gives the results of a study by positron emission tomography of the cerebral blood flow (CBF), oxygen-extraction rate (O2E) and oxygen consumption (CMRO2) during severe and prolonged attack of hemiplegic migraine. The salient facts observed are a high (CBF) in the brain hemisphere affected (ruling out the hypothesis of a persistent cerebral ischemia), together with a collapsed O2E (''luxury perfusion'') and especially preservation of the CMRO2 suggesting a decoupling not only between CBF and CMRO2 but also between CMRO2 and functional state of the tissue. Some time after the attack a new study showed the recoupling between CBF and CMRO2, but with the latter reduced in the affected hemisphere although the clinical and tomodensitometric state had returned to normal. These new observations should not however be improperly generalised to all migraines, given the unusual characteristics of the disorder in our patient

Ce travail rapporte les resultats d'une etude du debit sanguin cerebral (DSC), du taux d'extraction d'oxygene (EO2) et de la consommation d'oxygene (CMRO2) par tomographie par emission de positons au cours d'un acces de migraine hemiplegique severe et prolongee. Les faits marquants sont d'une part l'existence d'un DSC eleve dans l'hemisphere cerebral atteint (ecartant donc l'hypothese d'une ischemie cerebrale persistante), associe a un EO2 effondre (''perfusion de luxe'') et surtout avec preservation de la CMRO2, suggerant un decouplage non seulement entre DSC et CMRO2, mais encore entre CMRO2 et etat fonctionnel du tissu. A distance de l'acces, une nouvelle etude montrait le recouplage entre DSC et CMRO2, mais avec une CMRO2 diminuee dans l'hemisphere atteint alors que l'etat clinique et tomodensitometrique etait revenu a la normale. Ces observations nouvelles ne doivent cependant pas etre abusivement generalisees a l'ensemble de la maladie migraineuse, etant donne les caracteristiques inhabituelles de l'affection chez notre maladeOriginal Title
