AbstractAbstract
[en] Section 208 of the Energy Reorganization Act of 1974 identifies an abnormal occurrence (AO) as an unscheduled incident or event that the Nuclear Regulatory Commission determines to be significant from the standpoint of public health or safety and requires a quarterly report of such occurrences to be made to Congress. This report provides a description of those incidents and events that have been determined to be AOs during the period of July 1 through September 30, 1995. This report addresses three AOs at NRC-licensed facilities. Two involved medical brachytherapy misadministrations and one involved ingestion of radioactive material by research workers. One AO submitted by the Agreement States is included. It involved importation into the United States of a package having excessive radiation. No updates of previously reported AOs are included in this report. No ''Other Events of Interest'' items are being reported
